Hello Doctor, I Am 33 Years Old Women. The Problem

Question: Hello Doctor,

I am 33 years old women.

The problem I face can not be termed as excessive sleepiness as I getup on time in morning and hardly sleep during day time. But I fall asleep in sitting position or some time tend to sleep in standing position even while cooking in kitchen as well. I sleep unconsciously in sitting position often with my baby in my hands and don't bother about my baby knowingly it can be harmful for him as he may fell from bed while I sleep. I have no control on problem of mine.
I fell asleep while feeding him in less then 3 to 4 mins. This happens everyday while giving feed to my baby. Also, I hardly take time to sleep when on bed and don't even bother to lay on bed and fall asleep in sitting position only.

Also I want to point that this problem was not that severe prior having baby but it is increasing after I gave birth to my baby.

I have increased in weight after giving birth. Below are my weight and height details. If that can help you in determining my problem or may have any connection with it:

Height: 5'3" Weight: 78 Kgs

Please help me to get rid of this problem.

Thanks & Regards,

P XXXXXXX

Thorough evaluation needed...

Detailed Answer:
Hello Mrs XXXXXXX
Welcome to 'Ask A Doctor' service...
I've gone through your query in details...
I can understand your concerns...

Well, had it only been happening after the baby I'd first suggest getting some quality sleep during the afternoon (even an hour or two would be helpful)...

However, you've mentioned that this problem was not that severe prior to having the baby, means this was a preexisting concern (to some extent at least) in the past also...

Yes, your BMI is around 30.5 which is obese for your age group. Yes, post-delivery weight gain can happen, however, it can happen due to several other medical conditions also...

So, at this outset, I'd recommend the following:

Please get yourself checked up by an Internal Medicine specialist at first. Along with this, a thorough set of investigations need to be done (Complete hemogram, ESR, Arterial Blood gas analysis, LFT, KFT, Thyroid function test, serum Cortisol, FBS, PPBS, HbA1C, Vit D3 & Vit B12 Levels, 12 lead ECG and a Chest X-ray)...most of these are generally covered under a comprehensive Metabolic profile...

Please ask your husband/anyone who's there with you most of the time if they notice any recent onset snoring issues in you along with any drowsiness also (this is extremely important, and only the ones staying with you can answer this satisfactorily)...As there's also significant weight gain involved, in this case, some special investigations like a Sleep study should be done along with a specialist check-up (Neurologist or Sleep disorder Specialist)...

I'm not naming any particular disorder here, as these symptoms could be related to a wide range of medical conditions to even simple day to day stress...So, let's get these check-ups done at first which will definitely throw some specific light on it...Management can be done based on the findings thereafter...

Let me know if I can assist you further...

Wish you a good health...

Take Care
Kind Regards

Hello Doctor,

Thanks for your response. As you said I'll take time out to sleep during day time for an hour or two.

As per my husband I hardly snore. However, he also notice drowsiness in me sometimes while sitting on bed with me.

Please suggest from where we can get Sleep study done. I am not aware of any such doctor. So, please suggest for Sleep disorder Specialist in XXXXXXX area.

Thanks,

Brief Answer:
follow-up consultation...

Detailed Answer:
Welcome back,

Some drowsiness while sitting on the bed could be a normal bodily response reminding you to take time to sleep. While drowsiness during other activities or at other places (apart from the bed) would be more significant.

Yes, do take an hour or two to sleep during the day and observe how you stay thereafter...

Yes, it's a super speciality field, and only trained Neurologists can perform and assess this. You can refer to DR. MANVIR BHATIA at SLEEP APNOEA XXXXXXX (L-23, HAUZ KHAS ENCLAVE) for further evaluation. Appointments can be taken online on their website only.

However, sleep study alone would be inconclusive as for a complete check-up you should also get those investigations done as already discussed earlier.

Please discuss this and proceed accordingly.

Take Care
Kind Regards
