Question: Hello Doctor,

I am Female, 25 years, 58 kgs , 5'2 Ft. I have dark patches in inner thighs. It all started 3 years back. Whenever I have to work for long hours, while returning home by walk, friction between the thighs caused serious rashes and irritation. Later it has turned dark there after healing. I have Hypothyroid for the past 10 years and am taking 88 mg thyronorm. I also have dark underarms which I am not sure how it turned dark. But I had inflammation after a waxing session. I request you to suggest medications to lighten the darkness.

Hi,

Welcome to HCM...

I read history thoroughly and feel that you may be having fungal infection either Tinea cruris or candidiasis. Consult the dermatologist for the perfect diagnosis and proper treatment. Investigation like KOH smear test for fungus may be done to diagnose the condition.

If it is fungal infection, I would suggest as below...

- cap itraconazole 100 mg twice a day for about two to three weeks
- tab cetirizine 10 mg daily at night to reduce irritation and itching
- apply antifungal cream like sertaconazole cream twice a day
- avoid soap bath, use diluted dettol to wash the area
- keep the area dry
- wear cotton clothes
- improve hygiene and immunity
- avoid excessive walking to avoid friction of thighs

The darkened skin in axillae may be due to acanthosis nigricans or due to inflammation after waxing , which may be seen by your dermatologist and he may treat you accordingly

Dear Doctor,

You have mentioned tablet for reducing itchiness and irritation. But I don't have any itchiness there, just the dark patched remains where friction occured. But I sweat a lot.

Also, if the darkness in underarms is due to Acanthosis nigricans , will it lighten if I reduce weight?

Hi,

I can understand your concern for dark patches in groin areas. They may be be due to fungal infection, obesity,friction...etc. Take treatment for few weeks as suggested. Sprinkle ketoconazole powder in groin areas if perspiration is there.Try to reduce body weight. That would also help in lightening the the patches and also improve the acanthosis nigricans. Consult the dermatologist and physician personally to get real advice and treatment regarding your skin and body weight issues.
