Question: Hello Doctor. I have this issue of developing hives(not sure if it is what it is called) regularly. In the recent months the issue has started getting worse and I develop inflammation on my lips every 10-15 days. I had similar issue few years back and was diagnosed as urticaria. The inflammation normally happens in the evening and lasts till the next morning. Alongside the lips it also develops at several other body parts as well. I have not been able to track down any specific cause for this. Need your help on this. Attaching a picture of me to help you understand better. Thanks.

Get done detailed allergy testing.

Detailed Answer:
Thanks for your question on Ask a doctor forum.
I can understand your concern.
I have gone through the pictures you have attached.
This is classical of allergic etiology.
Best treatment of allergy is identification of allergen and if possible complete avoidance of it.
So in my opinion, you should get done detailed allergy testing.
This test will identify causative allergens in you.
At present take antihistamine (levocetrizine or fexofenadine) and anti allergic (montelukast) combination.
Apply mometasone cream on lips and on skin lesions.
Please let me know

Thanks for your response doctor.
No. I have not consulted any specialist for this neither do have any known family history of allergies.
Also, please let me know about the dosage of the medicines and cream mentioned. Do I need to make this a regular or use this up when the allergy occurs?
Thanks.

I can understand your concern.
Better to take combination of montelukast and fexofenadine once a day at night before going to sleep for 2-3 months.
Apply mometasone cream on lips and skin lesions thrice daily. Stop this cream once swelling and skin lesions subside.
Doses are as follows.
1. Combination of montelukast (10mg) and fexofenadine (120 mg) once a day at night before going to sleep.
2. Mometasone cream thrice daily.
Please let me know your weight so that I can guide about exact dose.
I will be happy to help you further. Wish you good health. Thanks.

Thanks for that. Firstly I would like you to correct my gender as Male (it shows up as female above). My weight is 60 and age 24.
The swelling has subsided now and is not constant but goes away in around 15 hours after occurrence each time. Also the other lesions are temporary and subside automatically.
Should I still take the medication and apply the cream?

Thanks.

Brief Answer:
Take only the tablet, don't apply cream now.

Detailed Answer:
Thanks for your follow up question on Ask a doctor forum.
I can understand your concern.
If you are not having swelling, then no need to apply the cream at present.
But keep this cream with you and start using thrice daily whenever swelling reappears.
You should definitely start oral tablet (combination of montelukast and fexofenadine) for at least 2-3 months.
I will be happy to help you further. Wish you good health. Thanks.
