Question: Hello Doctor.. My mother is not feeling good.. and little swelling all over the body and face.. so we got body checkup for her.. In medical reports she has TSH 50miu/mg anf fasting sugar 172 and little fatty liver.. in current corona situation, it is difficult for visiting hospital.. Please suggest

I have gone through your query and reports and here is my opinion.

As her TSH levels are high, it suggests that the thyroid gland is not functioning optimally and producing too little hormone.

Hypothyroidism might be the cause of her body swelling.

Usually, 50 micrograms of levothyroxine tablets are started to such a patient (once a day, to be taken empty stomach).

The thyroid profile (TSH, FT3, FT4) is repeated after 2 months and the dose is adjusted according to the values in the report.

Her fasting sugar is also high which raises a suspicion of Prediabetes or Diabetes.
She should avoid sugar, sweets, and foods with a high glycaemic index like rice, pasta, potato, melons, and white bread.

Such patients are advised to maintain ideal body weight, shed extra kilos, be physically active and have multiple small meals instead of three major meals.

A fatty liver is the reversible deposition of fat over the liver which usually does not affect the liver functions. Rarely, it might progress to hepatitis and cirrhosis.

For the optimum health of the liver and to decrease the level of the triglycerides in the blood, she should refrain from fatty and greasy meals, avoid junk food, keep herself well hydrated, have a rich fiber diet with plenty of raw vegetables and fruits and should take black coffee and green tea.

I would advise you to get her fasting blood sugar, fasting lipid profile and ultrasound repeated after a gap of 3 months and consult your clinician. By then, the global coronavirus situation should improve hopefully.

That is good that she is on medication for her high sugar,but the treatment regime needs to be revised as the sugar levels are not controlled optimally.

As I told you that Usually, 50 micro grams of levothyroxine tablets are started to such a patient.
The tablets are prescription drugs and needs to be taken empty stomach, in the morning.
