Brief Answer:
Your risk for HIV is negligible.

Hi,
Welcome to ASK A DOCTOR.
Thanks for posting your query.
The risk of HIV transmission through breast milk is less than 25% even after continuous feeding for months. Sucking breast milk one day carries only a negligible risk. Your other activities also carry only negligible risk as per CDC, XXXXXXX

Sir, thank you. In your experience does this episode warrant PEP as I am in the window.
Based on my reading, I understand that breast milk is more of a risk for infants. I have understood that adults are not at risk. Is your understanding the same. I also read that if you have mouth ulcers or sores (which I do not) it is a risk due to the breast milk.
Request you to clarify.

Brief Answer:
It is up to you to decide whether to have PEP or not
Detailed Answer:
Hi,
Welcome back.
Your activities are not having any risk for HIV transmission. Sucking breast milk once is also having negligible risk. I will not advise PEP for my patient in this situation. If you have disturbed mind, then it is better to start PEP and complete the course

Dear Dr XXXXXXX
Thank You for the reassuring advice. Currently where I reside, the cost of PEP is very much and I believe it has side effects. I consulted a local GP here
He mentioned as below
Thankfully the risk of HIV or STDs here is negligible. Although possible to spread HIV through breast milk it is usually from mother to baby because of the baby's immature immune system. Adult spread has not been reported. In this case I would recommend just screening for peace of mind in a month. However, if you are still very anxious then I will still dispense PEP for this but you have to consider if you feel it is worth the $2000+.
I responded as
Sir thank you. I trust you and would happily go by your recommendation. The money ,I can arrange but I understand there is lot of side effects which I wanted to avoid. In all fairness if there is in your experience anything to worry about then pls do let me know. Have a good day
He responded
Not a problem. I would recommend just screening in a month for this incident. Medically It doesn't warrant PEP
Based on your valuable suggestion and feedback from GP, I was thinking of avoiding the medication. Trust the decision is fine. I will do a P24 in 3 weeks time.
Regards XXXXXXX
