I have had a Kidney Transplant in 2009. In 2014, I was diagnosed with IGA HAAS -1. Everything was fine until two years ago my creatinine started fluctuating between 1.2-1.6. There was another biposy and other tests done. It was found that there is no rejection. And yet, my creatinine keeps fluctuating. In fact, it has now started increasing. The last two tests were 1.5 then 1.6. I also have protein in urine. It was 1.12 fifteen days ago, after increasing olmezest 20mg from once a day to two twice a day, it is now 0.47 but the creatine has risen from 1.5 to 1.67. My latest tac level is 6.8. Last time it was 4.2. Kindly guide. The reports are attached.

Detailed Answer:
Hi,

your previous graft kidney biopsy was suggestive of IGA nephropathy. It is probably a recurrence of your primary kidney disease in the graft kidney. it is a slowly increasing disease so do not worry. the recent increase in creatinine could be due to increase in olmesartan (olmezest) given to decrease protein loss in urine. that is acceptable. This medicine can slightly increase creatinine level but in the long term is beneficial.

if the creatinine keeps on increasing in next 2 readings then you should try to decrease or stop olmesartan under guidance of your doctor. Try to repeat creatinine every 15 days.

rest of the reports are ok. the repeat kidney biopsy is very good.

Thanks a lot Dr. Bansal. My recent Tac Level is 6.8. I would meet my doctor in a day or two but would you recommend lowering my Tacrolimus dose. Regards

Hi,
no need if all other things like bp, urine output and blood sugar are normal.

but some doctors may want to reduce dose by 0.25 mg to target level between 5 to 6.
