Hello Dr Basak. Hope Your Well. You Have Given Me
I ended up going here in Helsinki to see an ENT. They ended up doing Sjögrens A & B tests and also lip biopsy and RF factor. All ended up coming back normal and no Sjögrens.
I have now gone to the Helsink Antioxidant Clinic to try to get some help. However one issue came up and which is why i contacted you. I have been prescribed SALAGEN 5mg x 4 per day. If you recall i was taking Candesartan ( which i stopped taking). Now with Salagen its possible that there is maybe some conflicts between the 2 drugs. My own personal doctor is now on holidays for 2 weeks so was not able to consult with her.
I have been taking Salagen now for just under 2 weeks. Not much effects yet from this. However it was recommended i would resume now taking Candesartan 8 mg. But before i resume taking it could you kindly ok this and confirm there is not a conflict with these medications. From what I understand there may be some issues. Many thanks.
Christian

Hii. I hope you are doing good. Please take care of yourself which is the main priority of life. You were having dry mouth issues. They have given you salagen. Salagen is pilocarpine which is a very important drug. This is muscarinic receptor agonist. It can cause increased secretion of glands and will definitely help in dry mouth. But it also cause hypotension and vasodilatation in the body as its action may cause decrease in the heart rate and may eventually cause decrease blood pressure. Candisartan is angiotensin receptor blocker and it is used in hypertension. Its side effects can be decrease in blood pressure. So both the drugs can eventually cause decrease on blood pressure. But you have dry mouth which is a very significant problem. So you need to take pilocarpine. But at the same time you have to monitor blood pressure in home thrice daily. If any of the blood pressure gets too much low or you feel giddiness, weakness, black out then you have to stop pilocarpine for that day. It's all about judicial use and avoidance of drug whenever neeed. You have to be careful but you can start taking. Once you start taking both the drugs and monitor blood pressure you will get to know whether you can tolerate both of them. If not you can modify the dose of pilocarpine after advising by the doctor. Drink more water and food. Hydrate yourself to avoid hypotension. Start citrus fruits, milk, vitamin c and a rich diet. Hope you will be fine. Prayer for you. Please review if you have any queries.
Maybe you have some ideas. From time to time i have been having a strong taste of salt on my tongue, in mouth. Does this come from xerostomia / dry mouth? So is it typical symptom. Have been getting this a few times every now and again. Quite unpleasant. It appears to seem to be a typical symptom for a lot of dry mouth sufferers. Many thanks again.

Detailed Answer:
Hii. Xerostomia or dry mouth can present with salty sensation of taste. This is because saliva is not produced enough to get these taste particles mixed. The taste buds also don't get enough fluid to proper assess the taste particles. So patients often presented with salty or metallic taste. The only way to augment salivation beside pilocarpine is take vitamin a and c supplement and water. Please take fluid adequately as dry mouth can help bacteria to grow. You can take chllorhexidine gargle kn between to prevent infection. Please review if you have any queries. Take care.
