Question: Hello Dr.

I am wondering if there is a substance that will slow the metabolism of trazodone or nefazodone. I am very sensitive to the effects of the mCPP metabolite And I have a mutated gene for 2d6 (I metabolized mCPP very slowly).

I am looking to maximize the affect of the parent drug in either case (5ht2 antagonism) whereas mCPP causes agitation in me. I also take modafinil, which speeds the metabolism of trazodone/nefazodone.

Trazodone is metabolised by multiple ways and one of this is by CYP3A4. Inhibitors of CYP3A4 can theoretically increase levels of Trazodone but I am afraid this would not be very useful clinically.

Antibiotics like erythromycin, clarithromycin etc, ketoconazole, grapefruit etc are inhibitors. But should not be used clinically to increase the effects.
