Question: Hello Dr
I have suddenly developed nodular acne which are red with swelling and very painful on the right cheek. This has been for past 6 months. I have been treated by dermetologist with topical ointments and antibiotics from time to time. Even injections on the boils were given to subside them but they keep coming back. It's also with some kind of allergy as it's itchy and now small boil has starting coming even on eyelids. I am 42 yrs of age could be hormonal changes too as during menses they flare up. Kindly advise

Nodular acne require treatment with both oral and topical antiacne medications. You can consider taking isotretinoin if your acne keep coming back or is resistant to conventional antuacne treatment provided there is no hormonal imbalance. However, underlying hormonal imbalance if any first need to be confirmed with lab tests to rule out PCOD. In that case treatment is around correcting hormonal imbalance with oral contraceptive pills in addition to topical antiacne treatment.

Pain and swelling indicates inflammation. So you need to take oral antibiotics like either doxycycline or minocycline which are commonly prescribed antiacne antibiotics. Dryness and itching could be due to irritation to topical sntiacnr preparations that you may be using. You may rakd a oral antihistamine e.g Cetrizine 10mg once daily for symptomatic relief from itching. Isotretinoin is usually started in a dose of 0.3-0.5mg/kg/day however it may go upto 1mg/kg/day. It is a prescription drug that requires monitoring by lab tests and needs to be taken for many weeks or months.
