Hello, Exactly 3 Years Ago, I Was In The Emergency,

Question: Hello,
Exactly 3 years ago, I was in the emergency, I had a pain in my right testicale,. it was suspected a testicular torsion. After the operation, the surgeon told me it was little torsion not a big one. However I had in pain for months in upper right if the testicale where the stitches of the surgery was. I had many times blood tests and ultrasound and everything looked to be ok, no reason was found. When I asked here three years ago, the doctor told me that maybe a nerve was trapped inside the stitches and adviced me to live with it. Knowing that I don't have the left testicale, I lost it after 1 year of birth. Time passed by and I learned to live with it. I gained weight and I decided to go back to fitness and make some sports. Everything was fine, I was regular for weeks and I just woke up and the pain was back (no sudden pain or wrong excersie) even little bit more exactly where spot of the surgery 3 years ago(top right of the testicale) I even couldn't stand for 5 minutes, the testicale feels pain and goes above for longer time, I decided to take ibuflam and rest. After 3 weeks it go better but no away. I visited this time a very well know.doctor, he did for me a full test, by hand and ultrasound and he told me everything is fine, although if he press on this specific area if it feels pain alot. I told him about the possibility of trapped nerve, he told me it is already 3 years for surgery and stitches should have been gone, he advised me to continue on ibuflam.
The problem now I feel lost, I feel getting better but slowly and thats scare me, wouldnt be I able to play sports again or standup for long time. It has been already a month since the pain started to come back again. I only ask for possible explanation from your expertise.
Two things to add to my diagnosis:
1. I have problem that ureter of the right kidney has kink and it sometimes causes a problem, but I get check it regularly and shows no big problem
2. Before this pain thing started, I had alway a pain coming in the pelvic area and I thought it was appendix but doctor told.me not, now it looks like nerve problem or something related to right testicale

The types of pain am getting are always either
1. Burning sensation
2. XXXXXXX something is pushing or pulling in the same spot of pain in the testicale

Brief Answer:
It sounds like chronic nerve pain
Detailed Answer:
Hi Mostafa.hamdy1988,
I am sorry to hear about your troubles. The history and symptoms you describe sounds like chronic nerve pain (neuralgia) after the surgery, where a nerve has been damaged during surgery. There are some options remaining:
1. Tricyclic antidepressants in low dose may help relieve pain.
2. Gabapentin is another type of medicine that may help
3. Electroneurography is an examination of the electrical conduction of your nerves. This may help determine if it in fact is a "trapped" or "damaged" nerve that is causing you problems. If that is the case, minor exploratory surgery with nerve decompression may help.
Hope this helps. Please let me know if you have further questions.

Hello Dr Christensen,
Thank you for your clear answer. I know it is all over the internet but I just have more questions to clear the things up for me. I hope you can help me with them
1. Can it be the case that the stitches is still inside and not yet dissolved after 3 years (as I know part of the surgery that stiches are used to fix the testicale the right place)?
2. It means now, when I stand am applying pressure on the nerve, that's I can't stand for longer time however 3 weeks ago, I couldn't stand 1 minute without pain. Therefore the nerve might be healing?!. I just hope I can go back to the stage where I had pain only when I touch the area not to have by any movement.
Thank you

Brief Answer:
The stitches may be absorbed
Detailed Answer:
Hi again,
First of all, the stitches may have been absorbed by your body, but that depends on the suture material used for your operation. The operating surgeon can tell you what he used.
Second, there is a chance that the nerve is healing on its own, especially if you feel a spontaneous improvement in your symptoms.
Hope my answers are of help. Please feel free to contact me again if you have further questions.
