Question: Hello, I am 25 weeks pregnant and I have been on glyburide 2.5 mg for past two weeks. My sugar level without medication (but after diet changes and exercise) were as follows: fasting (93-102) mg/dl. A few days, it went up to 105/106 also 1 day without medication. My post prandial levels (2 hours after) are between 106 - 126 mg/dl. After starting glyburide, two days I woke up slightly sweaty and checked my blood sugar level at 70-72 mg/dl. My question is - do I need to continue medication at this level? I am worried that glyburide crosses placenta and I do not the implications of this on the baby.

I have gone carefully through your concern.
Gestasional diabetes is recommended to be treated with diet and physical activity and wait 2 weeks for the results.
Metformin and glyburide are not FDA approved but they can be used with consensus with the patient.
Insulin is the only medication approved for the treatment of Gestational Diabetes.
So, if diet and physical activity are not efficient, i would suggest insulin as recommended by your physician.

Thank you Dr. for your response. My sugar level without medication (but after diet changes and exercise) were as follows: fasting without medication - (93-102) mg/dl. 1. Are these levels high enough to be put on insulin? At what sugar levels, does it become harmful for the growing baby?

I ask because on two instances my blood sugar level dropped to 70-72 mg/dl and I woke up sweating. I understand that harm to the growing fetus can be caused from high as well as low blood sugar levels.

The normal ranges of blood sugar during pregnancy are;
70-95 mg/ dl fasting
Up to 140 mg/dl 1 hour after eating
up to 125 2 hour after eating
Over these levels insulin is started as they may be harmful fir the fetus and the outcome of pregnancy.
