Question: Hello, I am 43 year old male.
185 pounds, non smoker, non drinker. exercise all the time.
I was recently diagnosed with hepatitis A.
I had flu like symptoms, then turned yellow. went to the ER and they admitted me for 3 days.
my liver enzymes were 2,500 and 1,200.
They did a ULTRASOUND ABD RT UPPER QUADRANT on me and the results came back clear.
The liver results were
echotexture :normal, homogeneous
echogenicity: normal
surface contour :smooth
leisons none.

I have been doing weekly blood work for a metabolic panel and every week my enzymes have been coming down.
my values are currently
protein total 7.4
albumin 4.5
calcium 9.1
total bilirubin 2.0 [this was 21.3 when I was admitted in the hospital]
alkaline phosatase 73
AST 65 [this was over 1000 when I was yellow]
glucose 73
bun 12
creatinine 0.93
sodium 137
potassium 3.8
chloride 98
c02 26
anion gap 13
ALT 119 [this was 2,500 when I was admitted in the hospital]
GFR above 60 as thats as high as the hospital checks for.
prothrombin time is 12.8 [clot time]
I tested positive for ANA but then they ran a bunch of antibody tests to check for the autoimmune issues that can hurt the liver.
all of those antibodies came back neg.
including
smooth muscle ab panel [neg]
Lkm ab neg.
anti dna screen :came back good.
anti ena id panel
sm antibody -came back good
rnp antibody came back good
ssa antibody came back good
ssb antibody came back good
centromere ab came back good
scleroderma ab,igg came back good
ribosomal p protein ,igg came back good XXXXXXX 1 antibody came back good
chromatin antibody came back good

my CBC panel came back with everything in range...

Almost 8 weeks since I was admitted in the hospital and im back to workingout, working, and my color and eyes are 100% cleared up.
my high values should normalize with in the next 2 weeks I believe.

My big concern is I had a fibroscan while I had a lot of inflammation in my liver since my enzymes were in the low 300 range when I had this done.
they told me the test shows signs of F4 fibrosis.
ive read a lot about you will get high readings with the liver being inflammed but I don't know myself.

The 2nd ultrasound the liver doctor did in the hospital was a doppler ultrasound which showed mild spleenomagly
as well as it says
main portal vein is mildy prominent measuring 1.6cm [1.0 to 1.5cm is the normal range I have been reading]
also says non specific but can be seen with mild portal hypertension.

The 3rd ultrasound the liver doctor did was a vascular ultrasound of the liver.
this showed everything flowing good and in the right direction.

im so confused as to the ultrasound abd rt upper quad says everything looked good.
And all my symptoms with the onset of HEP A.
I tested neg for hep b and neg for hep c.
but they did a HEP A test to see if I was positive with a current infection of the virus or was it an old infection showing I had it at one time.
the results show I currently had it.

I have ran all the information by a outside liver doc and they suggest I do not have chronic liver disease.
up until feb 14th 2020 ive never had any sign or anything.
Im just not sure if I actually have heavy fibrosis / cirrhosis or is all the inflammation from the HEP A messing everything up.
My ultrasounds were all done at the time of my liver enzymes at 2,500 and 1,200
my AST was elevated way more than my ALT during this last 8 week situation.

I made a mistake when I told you my AST was elevated way more than my ALT.

I made a typo.
My ALT WAS IN THE 2,500 RANGE WHEN I WAS ADMITTED.
MY AST was in the 1,200 range.

not sure if that matters but I looked over my question I sent you and I noticed I made a mistake.

ALT was Higher than my AST.

I also forgot to note that I have not had any endoscopy even though I asked the liver doctor and she said no not at this time.
As well as I asked if she was going to go through with a liver biopsy and she also said no not at this time.

They asked me if I had any pain in the liver area under the ribs and I do not have any pain.

First thing I would like to say is that your doctor is overreacting to the problem.Going by the history given by you; You had a severe infection of Hep A.Things would take minimum of 8-12 weeks to be normal.
Hepatitis A is a viral infection and virus cause severe inflammation in liver leading to release of large amount of enzymes from liver cells.Things would take time.All you need to do now is to take a diet with
60% carbohydrates (complex) 20% lean proteins and 10-20% fats.
Your liver was injured so taking a light food would help it to recover fast.
Along with it milk of thistle could be taken ( It's a liver rejenuvator).
As per the history given by you I think that right now you are asymptomatic and your liver enzymes are continuously going down.
I think there is no need to panic and to go for tests in necessarily.
It will take time.
Eat very light and easily digestible food and give some rest to your body to recover.

I agree with you on the Hep A and recovery .
But the mild hypertension of the portal vein in the liver during the doppler ultrasound has me concerned because any time I look up portal vein I see the term cirrhosis of the liver.

And my fibro scan was apparently high the liver doc said, but I read that liver inflammation will give high fibro readings.

When liver cells are inflammed the pressure of fluid in liver is high ,This causes the pressure of fluid in portal vein to be high( mild pressure increase).
Portal vein arises from liver only.It's natural thing,As liver would recover pressure would come back to normal.

If a part of is inflamed then there is usually vasodilation(Blood vessels get dilated to supply more blood to that area but permeability also increases at the same time) which leads to leakage and accumulation of fluid in space between cells. When Hepatitis A causing virus attacks your liver it get inflamed and as a result the blood vessels gets dilated hence causing tissue edema and increase in pressure.
