Question: Hello, I am a 24 year old female with a 12 month history of fatigue and muscle weakness which appears to be worsening, as well as generalized pruritus. Blood results have showed high B12 up to 2000 over this time, and more recently low gastrin, high basophils and eosinophils.

I have gone through the attached report.
Muscle weakness, pruritis, high vitamin B12 levels and low gastrin levels suggest an underlying metabolic disorder.
High vitamin B12 levels are seen in liver diseases and kidney diseases.
I suggest you to get liver and kidney function tests along with ultrasound abdomen and pelvis.
Low gastrin levels can be due to hypotrophy of gastrin secreting cells in stomach or excessive acidity in stomach.
I suggest you to consult a gastroenterologist for upper gastrointestinal endoscopy and stomach biopsy.
Pruritis can occur due to dry skin, raised bile salts etc.
I suggest you to get urine bile salts tested.
Muscle weakness can be due to generalized weakness.

Kindly, consult the respective doctors for above mentioned investigations for further management.
