Question: Hello, I am a 32yo Woman, Normally fit and well, Non-smoker, occasionally drink alcohol, like to keep fit running and gym classes 3-4 times a week, BMI - 25
Over the past 3-4months I have been having episodes of symptomatic bradycardia with dizziness and light headedness and occasionally SOB and nausea, initially I was treated for low Iron levels but these symptoms have continued even though my iron levels are now normal. My resting heart rate is 55-60bpm but can drop to 40-45bpm even with minimal exertion. I have had ECGs showing sinus bradycardia at a rate of 41bpm (this was after a 10min walk to the GP surgery), I was symptomatic. On 2 occasions I have been woken with palpitations and found my HR to be 35bpm.
I had an echo that was normal and all blood results have come back normal too.
Yesterday while at work I had an episode of light headedness and dizziness, with a heavy feeling in my shoulders and both arms - I was checked in A&E - BP 126/79, HR 73, BM 5, Temp 36.6, 3 Lead ECG showed 1mm of depression in lead 2, but 12 lead ECG showed NSR with unknown T wave abnormality, Troponin level was 1. No nystagmus, no nausea, no vomiting, no headaches, I occasionally have difficulty focusing my eyes and sometimes find it difficult to take in information and concentrate. All of these symptoms are intermittent and I don't get them all at the same time.
I stopped taking Propranolol 40mg 2 months ago (started 10 yrs ago for tremors, not taken every day just when needed) and stopped taking Fluoxetine 20mg 8 days ago after reducing it gradually over about 6 weeks (was put on it in February this year for anxiety and depression after returning to work from maternity leave - now resolved).
My GP seems to be at a loss as to what is causing my symptoms and is unsure if it is cardiac related or maybe a vestibular disorder.
You thoughts would be greatly appreciated.

For this reason I would recommend performing a cardiac ultrasound and an ambulatory 24-48 hours ECG monitoring in order to examine your heart rhythm trends for a prolonged period of time.

Considering your ECG changes, it would be also necessary performing an exercise cardiac stress test.

You should discuss with your Doctor on the above tests.

Thankyou for your reply. I am awaiting a 24hr ECG in a months time. Do you think my symptoms are more likely cardiac related rather than a vestibular disorder or neurological problem then?
