Hello, I Am Having Symptoms Of What I Think May
I am having symptoms of what I think may be diverticulitis. I had blood in stool for a few days and still have a lingering pain in the left side abdomen just below the rib cage and nausea if i dont eat.
I am in a remote location and do not have access to a doctors office at the moment. I did have access to doxycycline for malaria prevention, and I took 200mg in two doses for 12 days and the bleeding stopped. Next i tried a clear liquid diet for 48 hours and it really didnt seem to help much. Started probiotic supplement daily.
I am fit work out 5 days a week, 39 years old, never had any other symptoms.
So my question is will just a diet modification and increasing my water intake eventually cure the symptoms completely? Is this diverticulitis? Do you have other recommendations of self treatment until I can go be properly evaluated?

4)Ever feel heart burn or acidity,gas,belching etc? I normally rarely get heart burn, but have been experiencing it more frequently since this started. When the initial problem was happening I had gas cramps in my abdomen, but after taking apple cider vinegar pills, it resolved it very quickly.

But the pain in the left side still lingers, still getting heartburn, and it still irritates my side to defecate. But no blood is present at all visibly.
12)Do you lift very heavy in gym? Yes, i lift heavy. I have had a inguinal hernia on the left side, that had open surgery and mesh put inside of it. But the pain isn't that low, it's much higher on my abdomen.

Hello there,
I've gone through your query in detail and as no investigative facility or doctor is available in vicinity and summing up your history presentation of symptoms etc I would say that 99% you either have a fissure or Hemorrhoid and clinically I don't think that it's diverticulitis.
Hard motion and constipation can cause excessive straining which in turn results in the injury of the colon and rectum(you have a history of severe constipation and you have written that some times you defecate once in 3 days)and this can further form such soreness and in longterm haemorrhoids.These Hemorrhoids are swollen veins in rectum which burst up when person strains while passing stools and hence blood is seen in stools.In some conditions Hemorrhoids are overlapped with fissure(minor tear in ano rectal canal) and chief complaint in fissure is pain/burning feeling after passing stools(it's moderate to severe) and may last from minute to hours.
Thirdly bright red signifies that it's originating from the lowermost part of the gut, i.e. rectum...had it been an internal bleeding ulcer or any other pathologies, it'd be dark brown/blackish/tarry as the acid in the stomach and upper part of the GI-tract interacts with the gastric contents.
Lifting heavy increases your abdominal pressure and hence blood flow to these small hemorrhoidal veins increases causing them to swell up and rupture.
A high protein diet lacking fibers and complex carbohydrate is a risk factor for hemorrhoids.
First and the foremost-You should start taking some stool softener and Laxative, i.e. Syp LACTULOSE 30ml twice daily and observe the progress (the dose can be further escalated up to 30ml thrice daily based on the initial response)...

Now after going through the complete thread it appears that your pain could be diverticular/Intestinal in nature.
As there is no access to any imaging technologies at your place therefore treatments would be done on assumptions which means that all three factors-
1)Constipation
2)Intestinal inflammation(as Tylenol is an anti inflammatory is helping you means inflammation is there).
3) Diverticulitis-Bacteria's are there in small villi's in intestines and unless you kill them completely problem won't go away.
So answer is yes with diet suggested and along with basic medicines this could be cured fully in few weeks but Anxiety too has a role in constipation which is the cause of inflammation in intestines.
Before giving a final prescription and diet plan please tell me that if you have anxiety issues or some depression etc is going on as you have mentioned you are on Wellbutrin.
Please mention the dose and duration of medicine?
After taking probiotic did you felt any change...Rate it on a scale of 0 to 10..0 being minimum and 10 being maximum effect.

8)Is it possible for you to arrange green leafy vegetables and vegetables like carrot radish lettuce etc. Yes, I have drastically increased my vegetable and fruit intake and it seems to be helping. I also have been drinking the 2.5 liters of water per day, and I think that really helped a lot.
9)Sir can you get any of these constipation relieving agent) at that place-
Lactulose
Polyethylene glycol
Lactigol
I can get the constipation meds, but only in a couple weeks. I'm relocating to a different spot soon, so I have to get situated, and then I can order them and see how it goes. In the mean time, I have been using apple cider vinegar, and it seems to help a lot too.

I would suggest you to continue taking Doxy 100 mg twice daily for 2 weeks more along with it take lots and lots of fibers and vegetable and fruits.
If possible get a stool softner quicky.
A pain medication like nimesulide 100 mg once can be taken.
Tell me how you feel after that in 5 days.
