Question: Hello. I am posting from my husband's account hoping you can help clarify an MRI I recently had.. MRI states: 4mm cyst in the anterior aspect of the left parotid gland. symptoms: hoarseness, trouble swallowing and feeling of "lump" in throat. previously seen by ENT prior to MRI . Area seen on EXAM also. Can you explain this issue and if a biopsy is recommended.? Thank You. Traci female 53 previous history of breast cancer with mastectomy of left side

I can understand your concern...

Well, first of all, the 4mm cyst in the anterior aspect of the left parotid gland doesn't actually explain those trouble swallowing, feeling of a lump in the throat. On top of that, there's hoarseness also (which actually comes from the vocal cords or voice box).

Considering your past medical history, I'd like to review your MRI report in details. If you're having any printed report, I'd request you to please upload/attach it here (there's a reports section at the right side of this web-page only)...

If you don't have the report, kindly try to find out if they've commented anything on the vocal cords, larynx, nasopharynx or Oesophagus (these are the potential areas which can give rise to such complaints)...

If nothing mentioned about those in this MRI, Since you've already seen the ENT earlier, I'd suggest discussing regarding getting a Video laryngoscopy as well as EGD done to look into it in details.

At this outset, with these set of presenting symptoms, I feel this is more necessary than the biopsy from the left parotid gland cyst.

If there's any previous history of acid reflux, gastric problems then you can try taking a PPI/antacid (eg. PANTOPRAZOLE/OMEPRAZOLE) once daily for a few days and observe how you stay (as longstanding Gastric reflux can also cause this)...

Please discuss this with your doctor and proceed accordingly...

attached is the report. I already had an EGD done. It was noted on the ENT exam as well. The ENT wasn't sure what it was he said it was lymph tissue. and was debating on sending me out for another consultation. My dr called this morning and said my test show nothing " did not even mention the cyst. I picked up report on my own . This has been present now for 2 MRIs and almost 1.5 years. This is why I asked about a biopsy. If report didn't come through I'll try again another way.

Well, this certainly needs further evaluation...Since EGD is already done, the Video Laryngoscopy should now be done for an in-depth analysis...

Yes, it may or may not be a lymph node. Since he's uncertain about the nature of the tissue, a fine needle biopsy can be performed to ascertain further.

However, before that, I'd suggest you discuss getting the Video laryngoscopy first, as it'll give a much detailed magnified look inside the larynx and vocal cords (this is with respect to the hoarseness and lump like feeling while the parotid cyst would be another finding).

Do also discuss regarding going on the PPI therapy as already explained earlier...

Yes, I've reviewed the report finally...but it states there's nothing unremarkable apart from the 4mm cyst vs lymph node (means there's some doubt about the nature of the lump) in the anterior aspect of the left parotid gland...

So, our previous discussion remains the same...Kindly discuss with your doctor regarding all of them (including the Laryngoscopy as well as the PPI trial) and observe the progress thereafter...

Regarding the biopsy, although not entirely clear whether it'll be beneficial but can certainly be done considering the inconclusive nature of the tissue...

Please keep me updated regarding the same...
