Question: Hello. I had unprotected sex 4 weeks ago (female /male vagina sex ). This week I had a fever for two days and swollen tonsils with with pus and swollen neck lymph nodes. Also night sweets a couple nights. Er doc prescribed penicillin which is helping (now on day 5 of treatment). Strep and mono tests are negative . Acute hiv? If not, what else ?

Sore throat with swollen tonsils, fever and cervical lymph adenitis is a very common infection. This can occur to anyone at any point of time. Sexual intimacy can also facilitate such infection. Culture negative for Strep or Mono is not an absolute one. If it is repeated by someone else in some other laboratory it may become positive for bacterial growth. Inadequate material or improper technique may result in a false negative report.
HIV acute infection will not respond to your antibiotic (Penicillin). So, your problem is most unlikely to be due to acute infection of HIV disease.
At the same time, as you had unprotected sex, we cannot rule out your risk for HIV/ STI also. Is your sex partner is known to you already and are you sure about his sex activities and HIV status?
Anyhow, if you are not sure, it is better to undergo for investigations for STI/HIV after the window period of these tests..

Common sore throat is caused by Bacteria like Streptococci, Staphylococci will respond to antibiotics. antibiotics can cure bacterial infections and the viral infection. HIV acute infection it is caused by HIV. It will not respond to our common antibiotics. It needs antiviral ART drugs. Most of the time it will heal spontaneously, but takes more than 2 weeks.
