Question: Hello. I have a question about absolute lymphocytes and HIV. I am having some symptoms which I do not know what to think or do about. I have had 6 HIV ab/ag test all come back non reactive the most recent after almost 1 year and an RNA qualitative test at 7 months not detected. I am really trying to rule out HIV from my mind. I have been dealing with this fear for almost 1 year now. My question is I compared my CBC from the beginning of this year and my absolute lymphocytes value has gone down by almost 100/ul from 1746 to 1659. I realize this is still within

Thanks for posting your query.
First of all, I confirm you that you are free from HIV Infection.
In our body the normal quantity of blood cells have a wide range of variations. For example, the normal Total WBC count will be ranging anything between 4000 to 11000 cells. Among which the lymphocyte percentage can vary between 25 to 35%. As per the total WBC count also the lymphocyte count can vary. In the presence of Any minor illness like common cold, fever, sore throat, diarrhoeal diseases to any specific fevers like Typhoid, Malaria, these cell count may vary. Without any obvious infection or signs also these counts can vary. These test reports are not having any diagnostic value, but can be taken as indication or additional information to support the diagnosis . That's all. So not to worry. Forget about the risky contact and carry on your regular routine.

Thank you Dr. XXXXXXX I know i should believe the test but because of my ongoing symptoms I am wondering if the tests are missing infection. I have lymph nodes I can feel in several parts of my body and loose stoll for several months. Also new symptom most frightening is I have lost almost all appetite for a week now. I was healthy strong man beginning of this year now I have lost a lot of muscle mass. I am afraid I have advanced HIV disease and the tests not finding it. Also my girlfriend having frequent headaches and some night sweats now.

Brief Answer:
Your symptoms are not related to HIV infection.

Detailed Answer:
Hi,
Welcome back.
You had multiple HIV tests. HIV test 12 weeks after the exposure are highly reliable and conclusive. The HIV 4th generation COMBO test (Ag/Ab 1 and2) tests are reliable more than 99%. You repeated the test 6 times (then it is reliable 100 %), that too even after a year after the exposure. You also had RNA qualitative PCR test. That is also a highly reliable test as far as HIV 1 is concerned.
There is no way for the HIV infection with you escaping all these tests. So forget that incident. All your symptoms can occur in various conditions other than HIV infection also. These symptom are there even before HIV era (before 4 decades). You consult a physician. He/she will examine you and investigate the cause for your symptoms and also for your girl friend's symptoms.. My sincere advice is that you also have to seek the help of a psychiatrist to get over the unnecessary tension. Nothing to worry.
Get well soon.
