Question: Hello, I have a question about how to self check for Hemorrhoids and what to do if I have it? For last 5 days, I have been feeling weird pressure near/around rectal area, and have been having some slight difficulty going #2.

I have read your query and here is my advise.

* There can be 2 types of hemorrhoids from surgical perspective.
Here are the necessary details for your under standing.

* External hemorrhoids

- routinely called anal skin tag
- you can feel its presence outside the anal opening with finger or mirror
- soft in consistency, same skin color as normal
- usually do not harm and are painless unless friction by hard stool or else
- occasional itching
- have to take care for avoidance of constipation and does not require surgery in
majority of the cases

* Internal hemorrhoids

- can be of different intensity ranging from 1st degree to 3rd degree
@ 1st degree is without symptoms and found accidentally by surgeon during routine
proctoscope examination

@ 2nd degree hemorrhoid pause discomfort during defecation, constipation, prolapse
when you pass stool and goes inside
- bleed on and off
- maintaining soft stools with laxatives, high fiber diet, analgesic, sitz bath and local
anesthetic gel are prime management concerns
- surgery in more discomfort or not satisfactory results with non surgical management
- you can only palpate soft tissue like mass when passing stool outside the anal verge
which goes inside on its own

@ 3rd degree hemorrhoids , often called prolapsed hemorrhoids

- severe in nature
- extremely discomfort giving situation, difficulty in sitting even
- bleed on & off unless thrombosed
- does not go inside on its own
- surgery is mandatory
- can see outside as bunch of grapes around anus

* My best guidance at this instance would be watch for the condition over period of time
with following measures

- high fiber diet
- bulk forming laxative as ispaghula husk or else
- avoidance of dairy fat, spices, junk foods, chilly
- avoid smoking if involved
- Sitz bath ( sitting in a tub filled with lukewarm water for 5 minutes each 3 times a day
particularly after passing stool )
- analgesic as tylenol or else after meals 2 times a day
- can indulge in light exercises, weight lifting should be avoided temporarily

Do follow up after 2 days or early as per the need.

Hello, thank you for the answer, I have followed your instructions carefully, and have had improvement. I think I have 1st degree internal hemorrhoid, as there is not external skin tag visible upon inspection, and there is not prolapsed hemorrhoids when stool is going out, so I do not think it is 2nd degree.

My main questions is, when will I be able to return to lifting weights? I think this issue was initially caused by heavy lifting during exercise. Is there a time table on when i can slowly return to that activity? Also, are their exercises I should avoid? Should I keep taking fiber and stool softener?

Thanks for the follow up.

* You can return to work out once total symptoms are recovered.
Main concern is to gradually raise the level of lifting weight as sudden pull or stretch
over the anal region is not advisable for your case.

* Intense pulling of abs is not advisable for a particular period of 2 weeks or so.

* Should continue high fiber diet, plenty of liquids on regular basis.
Stool softener for a period of 7-10 days on tapering dose method.
