Hello! I Have Issues With Acid Reflux For Years Now
My doctor suspects it may be eosinophilic esophagitis but not sure. Last month, after taking antibiotics for having a cold somehow I've noticed that the burning sensation when I swallowed was mostly gone! (may be also to diet changes as I reduced sweets, honey and chicken and spices).
However I still have an acid reflux issue and sour mouth at mornings and my esophagus still feels occasionally "out of place", sometimes burning and sometimes just irritated or "streched". Sometimes feels like having a lump on my throat.
Biggest Problem is that despite having a nexium 40mg per night for few months already (since January) I got worse recently . My voice is weakened and I feel like having a lump on my throat. Hard to swallow too. I've noticed the symptoms get worse when I workout on the chest area. I feel a squeezing/burning sensation on the esophagus. Also when I strech my back and neck my thoat lump feels worse. My thoat so feels a bit swollen.
Also not sure if it helps but I also suffer from eczema and it got much worse the last few weeks. My diet is pretty clean. I eat only grilled food, no spices, oatmeal, breaded oven baked fish, bananas, raisins, vegetables and chicken. Rarely some hot cocoa(no sugar) and some teas or garlic water.
I used to workout 3 times per week. I have a fit and healthy body. I don't drink or smoke. I meditate often and don't feel I have a lot of stress but I do work many hours in the computer. I have seasonal allergies but insuually control them with workout and with breathing methods (Wim Hof) and cold showers.
I worry that there is no improvement in my acid reflux and especially about my throat lump and difficulty ton swallow sometimes. I am out of balance. Yes, I feel stressed for it :( I want to start working out again with no problems and start eating food with no fear!
Please help. Thank you for your kind services to humanity. ❤️

Detailed Answer:
Hi,
Thank you for your query.
1. From your description the following investigations will help:
a. ImmunoCap/Phadiatop allergy test (all panels) and serum IgE levels.
b. Food Print (IgG) food intolerance test.
c. h. pylori antibodies (IgM, IgG, IgA).
d. Haemogram, AEC, ESR.
e. Vit B12 , Vit D levels.
f. Auto-immune work-up.
g. USG Abdomen
2. A change of medication to a combination of rabeprazole and levosulpiride for 2-3 weeks along with Mucaine gel is recommended. You may need a prescription from a local doctor. An anti-histamine such as Bilastine 20 mg once a day can be started.
3. Based on the response to medication and test results, it will be possible to find a long term solution.
4. Endoscopy / esophageal manometry will only be required if symptoms persist despite the above management. However, in patients with a change in voice and a globus sensation, I do a simple Video-laryngoscopy.
5. Please also let me know your BMI, daily water intake, any history of snoring/mouth breathing.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.
Regards

Hi,
Thank you for following up.
1. All these parameters being normal, you can start with the medications.
2. As mentioned before, based on your response to the medications and your test results it should be possible to achieve long term improvement.
3. As with many patients such as yourself, this is a tedious process and does take effort and time.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.
