Hello. I Have Pain That Is Prominent In My Collar
Question: Hello. I have pain that is prominent in my collar bone, shoulders, chest just below my collar bone and above it. The pain radiates down into my arms at times and its really sore if i touch my outer arms/shoulders. I also feel a tightness and pain in my feet, hips, knees and sometimes the back of my legs (calves). Plus irritating pain in my hands as well. I went to the ER back in November and they did all the tests, said nothing was wrong with my heart or lungs. But this pain became worse and had new symptoms just after I was sick at the beginning of March. I seemed to have made the pain in my shoulders/collarbone worse after i exercises last week. Its very painful somedays and uncomfortable but i want to get to the bottom of it so i can go on living my life and i cant go to the doctors right now because of COVID-19.

thank you for writing on healthcare magic.I have gone through your query and would like to ask you a few more details. Cause as of the history the site of pain seems to be from the shoulder or neck. I dont think it was related to the heart and lungs in the first place but one good thing, we have ruled out the serious causes.
1. Do you have pains in the night?Like unable to sleep on the side of the pain
2. Any tingling numbness in the hands?
3. Any history of dropping things from the hand or difficulty in lifting heavy objects?
4. Also I would like to do a couple of test: (i) Raise the hand above the head, take it to the back and see which movement is painful (ii) Rotation of neck. Left, right, up and down..and see if the symptoms worsen
With online help I can do so much only as we rely a lot on the clinical examination to sort out the site of your pain. If you can look up at youtube, the test for Supraspinatus tendinits and perform, it would be helpful. Cause the type of pain you have is either from shoulders or neck which we need to localize and reach the exact conclusion

3. I dont really lift anything heavy but i work at a pharmacy where im constantly opening bottles and staring down at the workspace.
4. When i move my hand over my head and move it back it feels uncomfortable and a little painful. it also makes a cracking sound in my shoulder. Same thing with my neck. it hurts and feels weird in my neck to rotate it. Especially the left side

Well the symptoms do feel more of shoulder related than the neck and looks like one of the tendon is inflamed. As per your history, it does look like Supraspinatus Tendinitis. It is very common condition and painful cause u always feel the nudge going on.
Confirmation always remains the clinical examination, but I will recommend a couple of tests which is possible you can get done
1. ultrasound of the shoulder
2. X Ray of Cervical spine(AP/Lateral)
Management is a bit tricky as lot of medicines dont penetrate the joint. But for now you can try with:
1. Tab Celecoxib 200 mg twice daily for 5 days
2. Local Gels
3. Hot water fermentation
4. Avoid sleeping on that shoulder, keep a soft pillow underneath it in case you sleep on your sides.
5. Physiotherapy if possible
6. Lastly if it doesnt work, we plan for a small injection but that only after confirming through ultrasound and x rays...
I understand this may be difficult to get done due to COVID situation, but at least we can start from here and see how much u get relief in a weeks time .
