Question: Hello! I just noticed in my magnifying mirror that I have a few black spots under four of my fingernails, and am attaching images of the same. I noticed that the one on my right index finger nail seems to be growing out along with the nail. After looking it up on google I think they may be splinter hemorrhages...? Of course it said on google they could be due to injury, nail psoriasis, or much scarier things like scleroderma and endocarditis (I told myself before I wouldn’t google these things!). Do you think that is what they appear to be and do you feel they are anything to worry about? I recently had an ekg and bloodwork, all was normal. I do have fibromyalgia, and have been taking more ibuprofen than usual due to achiness and sinus headaches. I also think I may have some nail psoriasis, as my fingernails tend to be brittle and chip at the ends. Thank you as always!!

These spots are splinter haemorrhages, most likely from trauma e.g nail biting etc. Besides you seem to have a fungal infection of the nail i.e onychomycosis. This could also be a reason for the splinter haemorrhages.

Thank you so much Dr. Kakkar for your informative and reassuring answer! I did some research and see that there are some fungal nail renewal products available over the counter...do you think it would be worth trying one out?

Hi.

Can try but I would first recommend a nail clipping for confirmation of diagnosis of nail fungus. If it is confirmed then I would suggest an oral antifungal e.g Terbinafine or Itraconazole as definitive treatment.
