Hello, I Just Received The ACR-Urine Reports Of My Father.

Question: Hello, I just received the ACR-Urine reports of my father. His Creatinine ratio is 118 mg/dL which is within the normal range, as per the report. But his Microalbumin/Creatinine ratio is a huge 6057.29, with the Microalbumin in urine value 7147.6. How is that possible and what does this imply. His earlier tests came back quite normal.

I have gone carefully through your query as well as both the reports. It seems that your father has long standing diabetes with poor control. Diabetes affects the kidneys and many other organs of the body. Gradually the filtering membranes of the kidneys get affected and start leaking. Initially microalbuminuria occurs (small amounts of albumin leaking out) followed by macroalbuminuria (significant amount of albumin coming out). As such the kidneys filter creatinine and during this course of action if albumin leaks out, a ratio between the two clearly denotes how much albumin is actually leaking. A higher albumin-creatinine ratio thus indicates a higher amount of albumin leaking with respect to actual kidney function. In your father's case, the kidney affection seems quite high and it would be a good idea to consult a nephrologist. The earlier report reflects different parameters and therefore cannot really be compared.

Sir, the eGFR tests done previously shows the creatinine levels at 1.15 and eGFR at around 68 percent. He has also been taking the following medicines for his BP and Diabetes ; Nexovas, Prominad and Gluconorm.

Are his kidneys already significantly impaired or can we prevent it by consulting a nephrologist as fast as we can?

Thanks for writing back. Yes, impairment of kidney function has begun setting in. It would be a good idea to consult a nephrologist but more importantly the sugar levels have to be kept in strict control.
