Question: Hello,
I just stopped taking spernalactone about 2 days ago. My husband and I are trying to have a baby, and I was under the impression that I only have to stop the medication 16 hours before trying to conceive. Last night we had unprotected sex and I am worried now that I haven’t been off it long enough. I was wondering what the time period is that I need to be off of spernalactone is and if I do become pregnant will there be complications?

Brief Answer:
You should be fine, as long as you do not use it again- you need your doctor's approval for discontinuation of Spironolactone

Detailed Answer:
Hello and welcome to "Ask a Doctor" service,

I carefully read your query and understand your concern.

Spironolactone is not recommended in pregnancy as it may affect the baby. On the other hand, I think that if you do not use Spironolactone now and on, the baby (in case of conception) should be fine. It is important to stop medications that we think may affect the pregnancy while we try to conceive as it may be very difficult to know if you are pregnant until you are at least 2 weeks pregnant (when the urinary pregnancy test comes out positive).

So, to conclude:
- You have already stopped Spironolactone so the baby, in case of conception, should be fine
- You should discuss the discontinuation of Spironolactone with your doctor as well (your doctor should approve this- depending on the reason why you used Spironolactone in the first place).
- you will need to take folic acid 400-800 mcg daily until you are 12 weeks pregnant or you may use prenatal vitamins daily (this is a routine recommendation to prevent some birth defects in the baby).

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.

Thank you so much this eases my worry. I was on a very low dose of spernalactone for acne. One last question I do take Lexapro always for anxiety and klonopin as needed but early ever take the klonopin. I feel like it will be vary hard for me to stop these medications. Is it ok to take these while pregnant?

Thanks,

Detailed answer to your questions below

Detailed Answer:
Hello and welcome back,

If you used Spironolactone for ance only then it is safe to just stop it. Be cautious if you have been using retinoids for acne, they do have real potential for adverse effects on the baby.

As for Lexapro and Klonopin, Lexapro is considered category C drug which means no known side effects are noticed during pregnancy but there are not enough studies or evidence to confirm this. So, it can be used if needed. In your case, I would say that you need to use it.

As for Klonopin, it is considered category D during pregnancy, this means there are adverse effects noticed when used during pregnancy, so in my opinion you should use it as rare as possible or not at all.

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.

Brief Answer:
Clindamycin is an antibiotic, while tretinoin is similar to retinoid

Detailed Answer:
Hello and welcome back,

Clindamycin is not a retinoid, it is an antibiotic which is safely used during pregnancy.
Tretinoin and retinoid are similar and should not be used during pregnancy due to severe birth defects it may cause. If a patient is using oral tretinoin (retinol) should stop it for at least a month before trying to become pregnant. If a patient has been using it locally (as a cream), she should just stop using it (creams are less dangerous than the oral pill).

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.
