Question: Hello. I’m a 49 year old male with height of 6’0” and weight of 157 pounds. I have GERD, borderline hypothyroidism, an anxiety disorder, and some struggles with chronic fatigue and sleep. I take Prevacid 30mg in the AM and Prilosec 40mg in the PM for the GERD. I take 75mcg of Synthroid daily for the hypothyroidism. I take 30mg of Prozac daily for the anxiety. I also take a supplement called Theralith XR to prevent kidney stones.

I just completed some annual blood work. My 12 hour fasting blood glucose level was 64 mg/dL, just under the reference range of 65-99 mg/dL. My endocrinologist suggested a 10 day session with a continuous glucose monitor (CGM). This morning I ate an RxBar (7g fat, 24g carbs, 12g protein) and two bowls of quinoa. The meal was filling. I finished eating at 11am. By about noon, the CGM showed a reading of about 196. I used a finger-prick blood test and got a result of 202 at that same time. Please see the attached image of the last 24 hours on the CGM. The rightmost peak is the most recent meal that I am speaking about now. I did some exercise immediately to help bring the number back down.

I have been reading that blood glucose levels above 180 are unhealthy. I’m concerned that I may have been experiencing spikes above 180 for some time now without knowing it. That and of course drops below 70 without any symptoms. Can you assuage my concern about what harm may have been incurred in my body for whatever period of time this may have been occurring? Is this highly unusual?

Your blood sugar levels around 200mg/dl are suggestive of Diabetes mellitus.
To clarifie the situation i suggest to do the blood sugar test (fasting and a two h after meals).
Another important test is HbA1C.The result >6.5%, confirm the diagnosis of diabetes.

P.S i culdn't see your recent result of CGM because i can 't open the attached report.

Thank you Dr. Xhardo. You mentioned having a fasting blood sugar test. But I noted in my original post that my 12 hour fasting result was 64 mg/dL. Doesn't that particular result seem inconsistent with a diagnosis of diabetes? I suppose I need an A1C test to help make any decision. Are you suggesting that any blood sugar measurement at any time over 200 mg/dL is almost certainly a diagnosis of diabetes? Thanks again.

The diagnosis of Diabetes is made if you have fasting bood sugar 126 mg/dl or higher and random blood sugars>= 200mg/dl in two separate days.
You have normal 12h fasting blood sugar result and random results suggestive of Diabetes Mellitus.
For this reason The HbA1C test can be necessary to clarifie your situation.

An A1C level of 6.5% or higher indicates that you have diabetes.
