Hello. My 5 Year Old Son Has Been Suffering From

Question:      Hello. My 5 year old son has been suffering from severe bloating, fullness, loss of appetite etc. He was diagnosed w/ Hashimotos a couple months ago and recently started levothyroxine. He has been tested for other AI diseases and has had bloodwork multiple times that comes back normal. I’ve been really concerned about his symptoms so His doctor ordered an abdominal ultrasound that showed mild fullness in left renal pelvis. I have attached his report below. -What does this mean? -How concerned should I be? -Does this resolve on its own or is medication needed? -How should I proceed in following up on this? -I also give him half a cap of miralax daily as per his gastroenterologist to help with his constipation. Could taking this daily possibly affect his kidneys? The bottle warns that people w/ kidney issues should not take it, so that’s why I ask. Thank you. ABDOMINAL ULTRASOUND HISTORY: R14.0 Bloating R10.9 Unspecified abdominal pain COMPARISON: There are no comparison studies available. Multiple transverse and longitudinal sonographic images of the abdomen were obtained. LIVER: The liver is normal in size. The liver parenchyma is homogeneous. No focal lesions are identified. SPLEEN: 7.4 cm in length . There are no focal lesions. GALLBLADDER: The gallbladder demonstrates no calculi, wall thickening, or pericholecystic fluid. BILE DUCTS: The common bile duct measures 2 mm. There is no intrahepatic ductal dilatation. PANCREAS: The pancreas demonstrates no focal or diffuse enlargement. RIGHT KIDNEY: 7.3 cm in length. There is no hydronephrosis, contour deforming renal mass, or calculus. LEFT KIDNEY: 7.4 cm and length. There is no hydronephrosis, contour deforming renal mass, or calculus. There is mild fullness of the renal pelvis. AORTA AND INFERIOR VENA CAVA: Visualized portions are normal in caliber. No ascites is noted. IMPRESSION: Mild fullness of the left renal pelvis. Hi dear,
I have gone through your query in detail and seen the ultrasound report
I understand your concern

But, there is nothing to worry much here. Such a finding is common in children when we do ultrasound for some reason. This means that the upper part of ureter on that side is a bit wider than the opposite side. All that is needed is to keep him under followup and do a repeat scanning after 6 months. Most of the times, the second scanning comes out normal. Only if the widening increases, we need to consider obstruction to urine flow on that side.

No treatment needed now, and absolutely no reason to worry. Laxative has nothing to do with this, but severe constipation can be a contributing factor. So, treating constipation is actually good for this.

Thank you for your response. If the 6 month scan comes back the same or the widening increases, what are possible things that can cause obstructions? Thank you for your time.

Brief Answer:
Functional blockage at ureter level

Detailed Answer:
Hi dear,

In that case, functional blockage(blockage without true anatomical obstruction) at some level of corresponding ureter will the possibility. Usually this occur at the level of upper(pelvi-ureteric junction) or lower ureter(bladder junction). A contrast imaging of the urinary tract will be needed then(MCU) to delineate this.

But, as I mentioned early, most likely this will disappear on repeat ultrasound, and no scope for any worry now
