Hello My 7.5 Months Daughter Is Having Rubbing/Grating Sound In

Question: Hello
My 7.5 months daughter is having Rubbing/Grating sound in her knees and we can feel bone movement when she stretches her leg in a particular position since past 2-3 weeks specially when she is irriated. ( we first observed this when she had viral Fever 2-3 week back other there was no such sound earlier).in Normal course she is playing moving legs , Rocking ( Stage before crawling), started siting and getting up on her own and sit.
We visited a Pediatrician Ortho and he first suspected that it's patella of knee that could cause however later when he observed the knee ( we irritated our daughter) he updated that this is not related to patella and could be some ligament( he requested X-rays )
I am attaching the prescription could you please share your views on what this could be.
1) Does 7 month old have knee cap ?
2) What could be possible cause of this movement.

One more question
Does Dimple in both knees point to any problem, He has examined and told us that knee cap is definitely there however it could be small, On internet we can see that such small babies doesn't have knee cap and it will develop on 2-3 years?

Brief Answer:
If she is not having any difficulty in achieving milestones-need follow up
Detailed Answer:
Hello, I understand you concern.
As long the baby is not having any symptoms - as long as the baby's development is normal, I do not think we need to worry about the grating sensation.
No need to go ahead with any investigation - or any treatment please.
Knee cap is a cartilage is present at birth and it will become ossifies by 3-5 years of age.
The possible reason could be an ossification process going or a normal physiological finding.

Brief Answer:
Dimples are normal - Sound could be due to calcification of the patella
Detailed Answer:
Hi...
Dimples are normal - Sound could be due to calcification of the patella. The grating feeling could be due to the calcification process of the patella.

Hello Doctor,
Thanks for the reply.
We visited the pediatrician Ortho for the followup visit and showed the X-rays.
1 ) On seeing X-rays he told that baby has a mild hip dsyplacia on the left side however he said nothing is required for the moment and we will followup with after 2 months and lets see the baby development. On Manual Examination earlier he cleared that there was no problem in Hips.
He told even if it remains then the kid needs to be sit in a harness to correct it and there can be no intervention till 3-4 years.
Please share your views
2) after seeing the knee x-rays he told he need lateral X-rays as bone in the AP views seems OK,he was earlier suspecting nail patella syndrome however told us Nails are clear and other associated symptoms are not there so it can't be that however he is saying that patella is small ,i am not understanding this point why he is saying this as in babies patella is still in developing stage.
Thanks for the reply.

Brief Answer:
Suggestions on patella
Detailed Answer:
Patella is still developing and this is true.
It is not true that patella is absent.
Patella is there - but it is not calcified at this stage and is in the process. So his view is right.

Sorry i am not able to correlate the answer with question.
As patella is still developing can a conclusion be reached that it's small or large.
For mild HIP dysplacia as pointed by him in X-ray do we need to do anything or it may go automatically with Baby growth.

Brief Answer:
Suggestions on hip dysplasia
Detailed Answer:
Even though X-rays are suggestive of mild hip dysplasia, as the clinician when your doctor feels that no active intervention is needed - we can wait.
But this needs follow up for sure.
Patella is not small or large. It will grow with age.
Initially you had asked whether a 7 month old will have a knee cap or not. For that I answered that patella will always be there and that - it will in the process of development as it is cartilaginous initially and later gradually get calcified and hardened in consistency.
