Question: Hello,
My dipsticks have recently gone from trace/neg(2018) to 2+ , 3+ and i was concerned. Had flex neg cysto in March/2019 but still conerned about increase. Had a ct urogram in XXXXXXX 2018 that was neg in response to trace rbc's and exsmoking history(35 pk yrs), 10 yr benzene/hydrocarbon exposure in 70's, son w clear cell Wilm's Tumor, 40 yr controlled hypertension. I've reduced working out and eliminated cycling(walking instead) to no avail. Asymptomatic and in otherwise good health. Thanks for all you do! XXXXXXX

You may have what we call idiopathic microscopic hematuria. This is a Benign condition wherein due to a hypothetical leakage of RBC in the urine patients would have RBC in the urine.

You have done everything that needs to be done with cystoscopy and CT urogram and everything came out fine.

We should just continue to monitor your condition with urinalysis and ultrasound every 3 months.

How reliable are dipsticks alone? I just had a microurinalysis showing trace, 0-5 for rbc's. Since this straddles the aua guidelines cutoff of 3 rbc's/hpf, I don't know id I'm above or below the cutoff. When would you repeat ct/mri if microhemsaturia continues? Thanks so much! XXXXXXX

Ct scan every six months for the next two years

Dipsticks are not that reliable because they rely on chemical reactions unlike urine analysis where in the cells are counted manually it is more accurate in determining the amount and severity of blood in the urine.

a 0-5 RBC per high power field could be considered as within normal. as for my patience i usually repeat ct scan after six months if there is persistently elevated rbcs in the urine together with a cystoscopy especially if there is gross hematuria. what is recommended is we follow up patience in micro and material for 2 years with regular check up with urinalysis ultrasound and urine cytology. a biannual ct scan is also recommended for the next two years. i hope i have answered your question satisfactorily.
