Question: Hello,
My grandfather has diagnosed with Infiltrating Urothelial Carcinoma, and the state is advanced. He has been in the hospital and done a transurethral resection, but the doctor said it's all over the bladder already. Due to his age and other chronic disease (diabetes type 2, high blood pressure, and peripheral artery disease, he had two heart stent surgery), the doctors have made decision that chemo therapy is not suitable for him. So far he had not other symptoms rather than urinate frequently, and he had left the hospital living normal at home now. My question is, I want to know if PD-1 inheritor (immunotherapy ) could work for him, considering his overall health condition. Because he is not receiving any helpful treatment right now, which basically waiting for the tumor to spread. I did some research on PD-1, and I learned that it does work for Urothelial Carcinoma, but the side effects are still need to be carefully considered for a 85 years old man.

Of course immunotherapy is the only therapy which can extend his survival, though response rates are low. In majority of patients, immunotherapy is tolerated well and i will advise to go for it.

I do need you to please further explain what do you mean by majority of the patients are tolerated well? Does it mean the side effect is not so significant to the most of the patients? And the second question is what are some of the common side effects of PD-1 inhibitor? Thanks Yucheng

Hello again. So i mean that side effects will occur in a few of patients only in the form of immune. Mediated pneunomia, hepatitis, kidney and gut inflammation and sometimes allergic reactions during infusion. These can be managed.

Thank you Dr. Sundriyal, if we did want to proceed to receive PD-1 inhibitor, where should I go to? The reason I ask this is because my grandfather is not in U.S, in fact, he is in China and China has just approved PD-1 inhibitor in this August. What do you think it is the best way for him to receive this treatment? The doctor in the hospital told me about PD-1 inhibitor and there were use to be patients who brought it from the U.S. or Hong Kong before approval. I am thinking maybe I can buy it in U.S. as well? What do I need to do then?

Great, I'm his grandson and I'm in U.S., that's why I was thinking U.S. as well. The only thing I worry is since China has just started to use this treatment, there might not be experienced doctor who has administered PD-1 inhibitor enough to know every aspects of this treatment.
