Question: Hello Sir,

attached ECG has been conducted in routine health check-up.
bifascicular block has been detected in ECG. require you help to understand what it/does not require any treatment/any precaution required/is any further test required to rule out any heart disease/what is prognosis.

Hello,
Its sinus rhythm with right bundle branch block, left axis deviation and lafb (Bifascicular block), No significant ST T abnormalities seen.
Now the prognosis depends on the underlying cause for these changes. Usually such changes are related to hypertension and have a good prognosis. However possibility of ischemic heart disease should be ruled out with echo and treadmill test if echo is normal, as patient is diabetic (which is a high risk factor for ischemic heart disease).
So usually has a good prognosis but needs further evaluation with echo and treadmill test.

Hellos Sir,

in case, CTMT & 2 D ECHO both are normal- would cardiac condition confirmed as normal or are there any other major risk involved in it which may effect individual in future.

looking age - will he be able to complete TMT
is there any other suitable test which will help to rule ischemic heart disease ?
is there any future risk of heart block- conduction defect ?

CT coronary angiography

Detailed Answer:
CT coronary angiography is an alternative test if patient is not able to perform tmt. CT coronary is more accurate than tmt in ruling out ischemia. If echo and Ct coronary angiography are normal then patient will have good prognosis. If CT coronary angiography is not available then try tmt or even stress thallium.
These blockages in Ecg are conduction blockages in electrical system of heart and will stay as it is in the future as well. They are permanent changes.
