Question: Hello. So I have been sick since Monday evening. At first I thought it was my period, which I don’t have often due to my IUD. I was cramping terribly, nauseous, and felt kind of constipated. I went to my OBGYN on Tuesday and my ultrasound came back perfect. Wednesday and Thursday, I’m not pooping much cause I have no appetite. Still very nauseous, I have chills, can’t sleep well, lower back pain, and more cramping. I ended up coming to my parents house and went to urgent care Thursday morning. She told me my urinalysis showed a UTI, and I may have a GI bug. I’m bleeding when I pee (might be my period), but I’m still very nauseous, in pain and worried it could be a kidney infection. I do not have a fever

Brief Answer:
PMS WITH UTI

Detailed Answer:
Hello, Thanks for the query to hcm,
The fact that you have chills and pain and urinalysis confirming urinary tract infection warrants for an antibiotic course for 7 days at least. Also the fact that you have bloating and constipation ( likely to be part of pms symptoms), thats the cause of decreased appetite.
Considering this being the first day of your menses( blood in urine), you can start the following regimen:-
1. TAB NITROFURANTOIN 100mg SR X 1-0-1 X 7 DAYS.
2. TAB RABEPRAZOLE-DSR X 1-0-1 ( BEFORE FOOD) X 7 DAYS.
3. SYP ALKASOL X 2TSF IN 1 GLASS OF WATER X 1-0-1 X 7 DAYS.
4 SYP CREMAFFIN X 2TSF X ONCE AT NIGHT X 3 DAYS ( FOR CONSTIPATION)
5. TAB COMBIFLAM X 1-0-1 X 3 DAYS ( FOR PAIN RELIEF)

STOP ZOFRAN AND AMOX-CLAV.
CONTINUE TAKING 10-12 GLASSES OF WATER IN A DAY AND YOU SHALL BE FINE .

Brief Answer:
PMS WITH UTI

Detailed Answer:
Hello,
Its not a kidney infection per se but a definite bladder infection, which if prersists long can affect kidneys. However the blood in urine or hematuria is seen in cases of bladder stones or ureteric stones or renal calculi or renal carcinomas, but considering the fact that its your. first day of menses, the collected blood may come out along with urine as well especially the early morning first urine.
Blood from urinary tract is to be considered and differentiated from menstrual blood, only if urine is blood stained each timed when passed during the day.
So take the meds as advised and you should be fine.

Okay thank you. How do I help myself not freak out? I feel like I’m making myself worse by freaking out more. I never get sick so it’s just strange to me. I’ve also been bleeding since Monday, not a lot, just a little and I was assuming my body may be getting used to my IUD, which I got inserted almost two years ago

Brief Answer:
PMS WITH UTI

Detailed Answer:
With an iud inside, sometimes the bleeding may last for 5-7 days even but if you feel that its in excess then you can opt for antifibrinolytics to control the bleeding.
Please take the meds as advised and follow up if needed.
