Question: Hello. So, this problem has been bothering me for a while now and has become quite annoying. It started in December 2018.
I started going back to the gym. On the first day back from the gym I got home, and I did PC muscle exercises which almost led to an orgasm (not first time but may have been a little harder clenching than usual). Nothing weird now, until that night I had sex (my gf used a butt plug on me, not the first time, and nothing out of the ordinary). But then I found that my refractory period was not there, my penis still felt sensitive, and I was able to have more orgasms (4) which was weird to me. After that I took a shower, but I was still sensitive (no erection but the feeling of stimulation on the head of my penis, or seminal fluid in the urethra. It is hard to describe but is like being close to ejaculation). It was difficult to go to bed that night, but I managed. Eventually next morning, the same thing.
This felling lasted for about 2 or 3 weeks sometimes a little bit less, sometimes almost unbearable and it was driving me crazy specially in public. Eventually it went away for about a month, but it eventually came back (although not as strong as initially). And since then is being coming and going somedays more, somedays less, somedays almost nothing. I am hoping to find a solution to this annoying problem, and I hope it’s nothing permanent.
I had a feeling that it was related to my left leg since it felt more sensitive than usual closer to my inner thigh and my feet. Its just a weird sensation that when I tense up that leg or when I try to move it to relax it, the feeling in the penis changes, but I’m not entirely sure. Around February I started to have some issues with my left lower back and left leg (slight ache). But I think this was due to me focusing on that leg too much. It eventually went away but I still have the slight feeling that it could be related to the penis due to some nerve connection or something.
Also, I had some testicular issues with the left testicle, slight ache and weird “pull” sensation when coughing, but doctor did a check and said it was related to a cold I just had. It eventually went away as well. It only happens if I cough to much or blow my nose a lot.
Some extra details:
- No UTI. After It started, I thought maybe an STD or something similar, but urine and blood test came fine. Was told prostate check was not necessary at my age or with this type of symptoms.
- Feeling (almost always) is gone in the mornings but it gets more noticeable as the day goes by.
- I don’t notice it when I’m hanging around with friends or my mind is occupied. But I don’t know if it’s really gone, or I just don’t notice it.
- Different positions sometimes help, like bending forward or stretching out my legs, or laying down.
- Having to use the restroom, peeing or bowel movement takes the feeling away for a little while.
- Walking usually helps
- Seating sometimes helps and sometimes it makes it worse.
- Bumping my knees (like when people are nervous) makes it worse.
- Sometimes the position of my penis (left, touching my thigh) might make it worse. Other positions (right or up) may make it better.
- Finally, it is much better (almost gone completely) when I have an erection, which I find weird. However, it does interfere with my sex life because I keep worrying that I will orgasm soon, leading me to last less in bed.
- I stopped going to the gym now, but when I was going, I found a similar sensation (although not quite exactly the same) when doing squats, leg lifts or leg raises. And some abs exercises.
I honestly don’t know if this is just psychological or if it’s a real problem, but it definitely interferes with my everyday life. I read about pelvic floor dysfunctions and those muscle groups, but I don’t know if the symptoms fit (either too much tension or muscles too weak). I don’t even know if its related to those muscles or maybe something internal since bladder, intestines and prostate are all so close. I just hope I can get a solution for this.

I forgot. Sometimes staying away from sex or masturbation may reduce the sensation as days go by. But sex or masturbation often, may bring it back. I also came back from summer holidays and i believe it was less severe when i was traveling with my friends. Maybe because i just wasn't paying attention to it
Another piece of info: I am a singer and we practice diaphragmatic breathing when singing. I add this because sometimes I feel that clenching my abs may diminish the sensation.

If there was any medical reason, it would've only increased with time.
You should not stop the gym routine.
Exercise will give you great relief from symptoms over time.
