Hello, Your Help Is Appreciated. I Received A Blowjob From
Your help is appreciated.
I received a blowjob from a 20 year old woman in Dominican Republic and after I noticed surface scarring on my head and bit of a deeper scar on the side of my head. This happened because of her teeth. She told me she had sex for money a few times but stopped 8 months ago. I didn’t know scarring is going to be left, didn’t really feel that.
- Does this scar issue increase a chance of me getting an STD? By how much? For which STDs in particular?
- Does this scar issue increase a chance of me getting HIV, Hepatitis C or Genital Herpes? By how much?
- Are there reports from people getting HIV when receiving oral sex?
- Are there reports from people getting hepatitis C from oral sex?
- How much more worried should I be because of the scarring?
Thank you very much.
Regards,
Sandro.

Detailed Answer:
Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.
* Oral sex has various dangers from clinical perspective depending upon
different situations as status of your immune system, associated co
morbid conditions, type of exposure and characteristics of partner in terms
of HIV and other diseases.
Here are some clues regarding your query.
1. Scar over penis head and side increase chances of genital herpes in
particularly if the person has active lesion in or around her mouth.
2. Various cases of HIV have been reported through oral sex though very
low, but more particularly if there is bleeding gums or mouth sore of the
job giver.
3. Hepatitis C transmission is very very low through oral sex, but possible
more particuarly if the person is HIV positive.
4. So far as your concerns are there, I would guide as
- Keep the genital clean with antiseptic liquid and dry.
- Drink more liquids to maintain hydration and prevent secondary bacterial
infection.
- For concern about the current scaring , kindly provide me further information so that I can answer your query more precisely.
1. Was there bleeding from the possible bite or tooth injury during the
process ?
2. Since how much time elapsed, you have noticed scarring and what is the
progress ?
3. If possible and no objection, visible scar if any would be best evaluated if
a photo attachment is provided in follow up conversation.
4. Do you have any other systemic disturbances apart from stress ?
Wish you best health always.

Detailed Answer:
Hi
Thanks for the details.
Kindly find further information as per the need.
1. Scar tissue itself does not increase the risk of getting STD because it is
the healing process of the body after wound ; what makes you at risk is
the initial event which took place before the scar appearance when there
were chances of transmission of various pathogens across the mucus
membranes of mouth and penis.
So far as types of STD are concerned, common ones being
(A) HIV : though oral sex is considered safer sex than vaginal sex, it is not
safest.
Data varies from institute to institute and various conditions for the
percent, but overall there are about 0.25 to 0.5 percent chances of
transmission through this way.
The great amount of reliability depends upon HIV status of the job giver.
(B) Herpes : average chances of 1.3 to 2.8 percent transmission of virus ,
more particularly when job giver has active infection in or around mouth
particularly.
(C) Hepatitis C : percentage depends upon the viral load of the pathogen
but very few chances.
2. There are other infections as anaerobic bacteria from the oral mucosa
as Peptostreptococcus , Acinetobacter or others which can give rise to
foreskin infection or inflammation.
As there are no signs of active infection as per your description, I would suggest to get a clinical evaluation with your family physician for primary care and if he or she thinks then you may get lab reports for particular infection.
Hope this makes query more sound and clear.
Thank you.
Take care.

The scarring happened DURING blowjob because of the teeth - so does that increase the chance of me getting an STD? If yes, which one? And... by how much % increase?
So if a blowjob giver has HIV the chance of getting HIV for a receiver is 0.25 - 0.5%? What is the chance of transimittion of HIV for vaginal sex that a man gets it?
Thank you.

possible genital herpes. likely need of antiviral therapy.
Detailed Answer:
Hi XXXXXXX
I have evaluated your images through given links.
* Tiny red dots and spots at distal part of penile shaft and glans penis
indicate possible early sign of genital herpes.
* It is recommended to consult your family physician for primary evaluation
clinically and get necessary prescription of medications as per the need in
terms of anti viral and other supportive care depending upon the lab data
which he or she will decide to guide you.
* Primary home care would be application of aloe vera gel, but its soothing
and healing purpose, you must aim at the precise diagnosis and
management in accordance to that only.
Wish you best recovery.

Hi
* The presentation of lesion of herpes vary in many different contexts so far
as clinical symptoms are concerned.
* There can be different non specific lesions, but history of oral sex and
spots suggest more possibility of herpes.
Bacterial infection is generally ruled out as there is no fever, associated
systemic symptoms.
* So far as data for the herpes and other diseases are concerned, it is
updated by center for disease control society of XXXXXXX from time to time
and involve multiple parameters of age, sex, race, region and multiple co
morbid conditions co relation.
There are 2 common types of Herpes type 1 - oral herpes and type 2 -
genital herpes ; the prevalence of 47 % and 10.5 % in age group of 14-49
years in USA.
* I would request you to get one time physical examination with your family
physician for sure.

- He prescribed me with Valacyclovir on Tuesday and he said it will take a few days (3-4 days) that it will go away. It’s been day number 3 now and there is no difference and a few additional spots appeared on the skin below the head.
When will it go away with the current inflammation that you see and taking Valacyclovir twice a day as prescribed.
Please do not just give a generic answer but give it for my cases.
- After taking the pills I have diahrrea, in certain parts of the day I feel more tired but more importantly every once in a while my stomach will start hurting (every 1 or 2 hours) for 10-20 seconds and then it will go away.
I know stomach upset is part of the side effects but should I be worried since this keeps persisting.
- Can I to out and drink alcohol while on this medication? 1-3 drinks or can I have more?
Thank you, I really appreciate your help XXXXXXX

Detailed Answer:
Hi XXXXXXX
Welcome for follow up.
* So as per our last discussion, diagnosis is confirmed by your consultant in
terms of genital herpes.
* Healing of genital lesion of herpes vary individually, but as per my clinical
experience, after starting of systemic anti viral medications as you are on
Valacyclovir ( best molecule for current condition available ),
approximately period of 2-3 weeks is average for resolution of genital
lesion, in some cases it may take even bit extra time.
* In some of the cases ( as in yours ), due to effect of viral killing by the
drug, the blood level of virus particles raise and there may be more
number of dots on genitals, which fade gradually over period of time ; so
it is not so that infection is spreading or not under control.
* The drug is a Guanosine analogue, mechanism of the drug is at DNA
synthesis level of the virus via affecting DNA polymerase enzyme causing
chain termination.
* So do not predict that dots will disappear within 3 days of medication as
per the narration of your doc, just continue medications and have
patience, it will surely be disappearing once the body system has adopted
its clearance by anti viral medications.
* Coming to handling the side effect of medicine, common GI upset is quite
normal, but following measures will help you to get through the same
- intake of antacid as ranitidine ( 150 mg ) morning and evening on empty
stomach
- pro biotics with lacto bacillus or else also help replenish disturbed GI flora
- dietary measures to avoid spices, non veg if consuming and addition of
fresh yogurt for natural flora
- alcohol consumption is not medically appreciable due to alteration in liver
enzyme pathway as well as disturbed GI level at present
- local application of aloe vera gel for soothing effect and faster healing
Hope this information will help you surely.
Wish you best recovery.

How are you today?
I appreciate your help so far and I would appreciate further advice.
As I said, my current side effects sre diahrea, tiredness in the afternoon and stomach upset - I would feel quite uncomfortable for 10-30 seconds a few times a day.
Today when I took my second does however the unease/upset lasted for 45 minutes and it was quite painful i guess too, definitely uncomfortable.
I can’t call a doctor because I am from Europe and I’m in the US at the moment. I have no kidney disease, hiv or weak immune system or take other medications. I did take some creatine yesterday and today however.
- Any suggestion?
- Are there any dangers when something like this severe happens?
- Should I switch a medication and if so which one?
- How many times a day do you recommend I wash the penis?
- How many times a day should I apply aloe vera?
Thank you very much.
Respectfully,
Sandro.

Hi XXXXXXX
Welcome for follow up.
* Uneasiness usually indicate possible low blood pressure from lack of
hydration and associated stress as well.
- Transient feeling of the same is not problematic, but persistent uneasiness
may be of warning sign of underlying constant low blood pressure.
- The current medication has to be continued to kill the herpes virus, what
most we can do is to adjust with the gastric upset.
- Penis cleaning may be sufficient for twice a day, as the main job is of
systemic anti viral medications rather than local wash.
- Aloe vera application 3 times a day.
- Maintain hydration with plenty of liquids.
- Add fresh yogurt or commercially available probiotics for control of
diarrhea.
- Avoid stress and panic as you can not change the diagnosis and condition
by keeping extra stress.
- Taking multi vitamin capsule or tablet will also boost up the immunity and
overall well being.
Hope this information will help you.
Have a great day.

Ok, understood, thanks, that makes sense.
I would really appreciate it if we can answer this questions in detail as well.
- So if I mantain my hydration I am in no danger?
- I didn’t take the pill yet today but I already woke up with stomach upset and occasional pain. Afraid to take a pill so it doesn’t get even worse.
Any advice? Should I just push through.
- So I can’t change the some other medication?
- I felt like things were going good and there might have been a mild improvement but today I think it’s actually a little worse and some new spots appeared. https://ibb.co/NShtvy4
When do you think this will disappear based on my case? It’s been a week.
- What if this doesn’t go away in 10 days and I am out of pills?
- PLEASE confirm if I am taking the correct Aloe Vera and that Ingredients are ok to use.

Hi XXXXXXX
Welcome for follow up, I have gone through your newer attachments showing penis image and aloe vera content display.
* Maintaining hydration is to avoid fluid imbalance and flush kidney in best
way when there is underlying infection condition.
* You are in danger when you are not taking medications in correctly
prescribed doses and skip them for fear of stomach upset.
* In the battle of war against virus, if you want to win, you have to be on
front lead coping with all minor GI upset and others ; skipping dose just
delays your recovery and you are harming yourself.
* Red spots are actually fading process of skin lesions produced by virus,
which is natural tendency to get bigger, fainter and then disappear.
The time as already discussed in my previous answer is usually around
2-3 weeks depending upon individual response.
* Do not think of what will happen if it does not go away by 10 days, always
Be positive, they are going to disappear with period of time. Your negative
thoughts are your enemy for sure.
* Changing medication is not an option as there are very few number of
medications available for viral conditions.
* Taking Imodium will do additional abdomen cramps in such case and not
recommended, as the issue is GI flora upset and probiotics are only
soldiers for this condition.
* My all time message would be stress control, do manage to engage in
some YOGA practice for about 20 minutes each, which will really help you
on long term basis.
* Coming to Aloe vera content, it is perfect and use for twice a day.
The other aloe vera was not harmful because the ingredient minor portion
of preservatives do not get systemic absorption or pause local problems
with very tiny concentration.
* Just adhere to your medicine regimen for best and earliest recovery, the
issue is to handle GI upset and stress associated with guidelines provided
at my best knowledge.
Wish you best recovery with patience.
