Question: Hello and goodmorning.. I am Farha from XXXXXXX I am 34 yrs old. Offlate I am feeling severe sensation in both my left and right hand, right hand being the prominent one. I got myself checked with a neurologist and he advised me to get a nerve conduction test. Accorfing to the report I am diagnosed with bilateral carpal tunnel syndrome. Currently I am on medication and the doctor has advised me to continue with PREGAREST M (1/2 TAB for 20 days and 1tab for 90 days). Tab ATNURON PLUS (1 TAB after lunch for 60 days). I got myself checked with the doctor on 27/1/20 and from the same day started the prescribed medications. But i didnot get any sort of relief and during the second consultation the doctor added another Tablet (SHINE 10 mg at bedtime to be continued). After taking all the medicines I am not having the slightest of relief and my right hand is giving me a tough time during midnight and early morning hours. I do feel numbness in my left hand also but my right hand is giving me more problem. There is a sort of tigling effect and i feel the prick of needle in both my hand. Further there is also pain and it extended till my. Right arm.
As It will not be possible at my end to get in touch with you physically for consultation due to travelling constraints, so I opted for online consultation, hoping you could help me in a better way. Awaiting for your valuable advice and suggestions.

Cheers and thanks..

Brief Answer:
Findings are consistent with a diagnosis of carpal tunnel syndrome.

Detailed Answer:
Hi Mrs XXXXXXX

I have noted your complaints and nerve conduction report findings. I agree that the diagnosis is carpal tunnel syndrome, where the median nerve gets compressed at the level of wrist.

Initial treatment is with medications, such as pregabalin or gabapentin capsules. The recommended dose for pregabalin is 75 mg twice daily and for gabapentin, 300 mg twice daily. Medicines should be continued for one month to note any benefit.

About 70% people respond to these medicines. Those who do not respond, are referred for a minor surgery at wrist area, to relieve the compression of the nerve.

Best wishes,

Dr Sudhir Kumar MD DM

Thank you for getting back.

Please give yourself 4 weeks time to assess whether medicines (pregabalin) is working or not. If there is no response or inadequate response, you can get the surgery done, as mentioned in the initial reply.
