Question: Hello, are elevated WBC in urine (11-25), (25>) normal for someone in their 80's? How about positive Leukocytes?

Thanks for query .

Based on the reports of routine urine analysis that you have posted elevated WBC detected on microscopic examination in a elderly female always suggests infection which is very common in elderly females due to following factors
1) Hormonal Imbalance
2) Chronic Cystitis
3) Senile urethral stenosis causing incomplete emptying of bladder resulting into significant amount of urine to remain in bladder all the time (PVR) which is a predisposing factor for getting UTI.

WBC or Leukocytes more than 2-3 per HP field are the dead leukocytes that die while fighting against the infective process in urinary system .

I have no doubt in what you are saying. Can you help me understand why my grandmother's doctor would state the following after we asked him about the results of the most recent urinalysis?

Doctor: "The urine culture results show no evidence of an infection."

Patient: "LEUKOCYTE ESTERASE, UA: Positive+"

Doctor: "Yes, the leukocyte esterase was (+), but that is not indicative of a urine infection. The confirmatory test is the culture, which shows insignificant growth."

Patient: "Do I continue taking the Cephalexin?"

Doctor: "You may stop the antibiotic that was originally prescribed. It is common practice to prescribe antibiotics when the initial urine studies show evidence of possible UTI. If the culture returns with a negative result, we recommend stopping the medication soon after."

Thank you, Dr. XXXXXXX My grandmother has two cancers right now. One of them is triple negative breast cancer. The other is unknown at this time. Three pathologists have reviewed it and they can't come to a conclusion. Chest CT showed nodules in or around lungs. She also has a lump in ABD and I suspect she has either cancer of the bladder, kidney, or the urethra and that one of those have spread to lungs vs it being lungs that has spread to ABD and who knows where else.

In six months time, she lost 16lbs which was 12.4% of her weight. Dysphagia, coughing, lower back/side pain, burning/pain while urinating, frequent, often little urination at a time. A whole list of things that should have been clues to at least further investigate to figure out the reasoning for these things. It's only because of the lump she felt on her breast that initiated a biopsy which showed breast cancer. I simply don't understand why preventive imaging was never done. Something that I think about all the time is that if someone has a cough and they go to the doctor, they send you home with cough medicine. Why don't they try to figure out why you have a cough?

Thanks for detailing the facts .There is a term called Non Bacterial Cystitis where patient has all the symptoms typical of UTI but there is no positive evidence of bacterial infection in urine culture .This is very common in elderly females. I have already highlighted the reasons for the same in my earlier reply..

As regards investigations to establish diagnosis of her lump in abdomen you need to consult qualified Oncologist and get her CT scan or MRI of the abdomen done .
