
Hello Doctor, 6 Weeks Ago, I Woke Up With A

Question: Hello doctor,
6 weeks ago, I woke up with a red patch on my nose, the patch was small and non itchy , I applied hydrocortisone 1% and the redness did not disappear so 10 days ago, I visited the dermatologist, he told me that it is an early stage rosacea. He prescribed 1% Ivermection cream (topical). I applied the Ivermectin for 6 days but the redness get worse and my nose become itchy and burning. My eyes are burning as well, sometimes my eyes start tearing with no stop. I applied eye lubricant and got relieved. I visited the dermatolgist again 2 days ago and he told me to stop the Ivermectin and he told me that is probably an allergic reaction to the Ivermectin and he told me that he will not prescribe systemic medications because I do not have any papules or pustules and the skin is smooth and not rough. He prescribed Brimonidine Tartarate 0.33% Topical gel for the redness. But to be honest I have not applied the Brimonidine Tartarate 0.33% gel for 5 weeks (once daily)yet because I know that its effect is temporary and I am scared of rebound redness when I stop it.I noticed that I have metronidazole 1% gel at home so I decided to give it a try. It did NOT irritate my nose, however, when I woke up, I noticed some dark spots on my foregead above my eye , I am not sure if that dark spots are from metronidazole or rosacea or something completely unrelated.so I am confused now, what shall I do? shall I leave the redness without applying anything or follow the dermatologist and apply the Bromindine Tartarate gel only without applying the metronidazole?I emailed all the photos to YYYY@YYYY because there alot of photoes and for some reason. I can not upload it here.I would like also to add that I started feeling minor pain in my right shoulder and right elbow. I am not sure if this could be of significane to the skin issue I am complaining of. Feel burning in my face. ALSO that dark spots in the forehead looks like purpura. They are little raised and not itchy.Those joint pains I experienced before but without skin redness or burning so it could be something of no significance, it is just I prefer explaining all what I feel at the same time. I want to stress that i had no itching or burning except after going to the dermatologist, trying Ivermectin and the metronidazole. I am sorry for writing too much but I believe all what I wrote is important information. Thanks

You seem to have acne rosacea - redness on tip of nose along with acne bumps on your forehead. I suggest you to use Clindamycin 1% gel, twice daily, for the pimples on your forehead. In addition I suggest you to take Doxycycline 100mg orally, as it would help both acne as well as redness. You may continue with metronidazole 1% gel, twice daily on your nose. Metronidazole gel is a much better option than ivermectin cream. Your neck and shoulder pain seems unrelated.

So this purple spots on my forehead is rosacea??

I mean the purple spots above the eye that looks like rupture capillaries , is it acne rosacea?

I looked again at those purple spots on my forehead, they look like ruptured blood capillaries under the skin. As I mentioned before they look like purpura. They are not raised , not itchy. Does Rosacea cause that? I googled and found that Kaposi sarcoma looks like that but my family dr who is not a dermatologist said no it is not kaposi.
