
Hello Doctor, From Last One Month Whenever I Lie Down

From last one month whenever I lie down I get a throbbing pulsation on my skull, if I lie on side while sleeping can hear pulsation sound near ear temples when I close my eyes, this is not painful and goes away when I am standing or walking etc, I also experience spinning feeling when I close my eyes and goes away when I open my eyes.. 3 months back my bp was slightly at higher side so I was prescribed with potassium losartan, checked my average bp last month it was 124/85.. Sometimes watching TV or when I sit still I get this feeling and discomfort so I engage myself in some activities to divert attention, the throbbing and pulsating feeling also can be felt in back side lower back and neck I can see pulsation in upper abdomen also but nothing is painful, please suggest if these r vertigo or migraine symptoms, are there any medication that can be taken temporarily till I can see a GP physically when coronavirus pendmic is over in few weeks time

Based on the history you have shared, these symptoms can be early signs of benign paroxysmal positional vertigo however possibilities like vestibular migraine should be ruled out. For that matter otolaryngologist review may be needed but you can use betahistidine as well as metaclopramide if symptoms gets worsened. You can see Eplys maneuver from YouTube and practice yourself at home meanwhile.
Also with such pulsations baselines lipid profile and vasculitis profile for any clotting or inflammation of blood vessels will need to be ruled out. Carotid artery Doppler ultrasound may also be needed in this regard. Also neurological examination for cerebellar signs may be needed once you get to visit GP.
These are just possibilities which needs evaluation but doesn't seem to be urgent needing any ER visits at this moment.
Keep track of blood pressure and keep good hydration, limmit salt intake, and supplements for B12 and folic acid may be used alongside

You can easily find supplements for B12 and folic acid in many OTC multivitamin supplements and almost in every one the dosages are based within limmits of daily intake and doesn't cause intoxication of these Vitamins. Also since B12 and folic acid are water soluble excess isn't likely with good water intake gets flushed with urine.
However hypervitaminosis D is a renowned entity and it seems that you have had sufficient replacement already as these symptoms can be due to excess of Vitamin D as well which is fat soluble vitamin and it's excess can be likely. So once lock down is finished you can have it's levels checked but at this moment it's better to stop it.
Using betahistidine may need prescription most likely but metaclopramide is usually sold OTC but again despends on state laws and local country laws which may varry across the world and since it's global platform its difficult to predict that. Wishing you a speedy recovery. Regards
