I am a 32 yr old male. I am affected with phimosis which I came to know of only now. My urologist has advised that I do circumcision. We are planning for a child for quite some time now. As per my semen analysis, my sperm morphology is 3% as per Kruger method, with rest being normal.
Will circumcision help my chances of becoming a father ?

Morphology of the sperm is unlikely to be changed after circumcision.

Hello,
Based on the facts that you have posted you seem to have Phimosis and your routine semen analysis has revealed morphology of the sperms to be 3%.
I shall be happy if you can post me more details to help me in making precise comments
1) Are you able to retract your foreskin completely over glans penis ?
2) Do you have difficulty in penetration of vagina during intercourse ?
Circumcision is generally advised if one has difficulty in penetration of vagina or pain during intercourse
Truly speaking morphology(Size and Shape) of the sperms is not governed by the status of foreskin .So it is unlikely to improve after surgery of Circumcision .
Following general measure will help to improve morphology of sperms to some extent .
1) Practice regular exercise for 45 minutes followed by meditation for 1/2 an hour in the morning.
2) Take high protein diet rich in vegetables and fruits and Vitamin A,C,D,E.and Zinc
3)Take anti oxidants like Almonds 5-6 everyday..
4) Avoid alcohol and smoking..
In view of your morphology being only 3% which is very low compared to normal value of 14% for effective conception I would suggest you to consult qualified infertility specialist to explore the possibility of pregnancy with the help of newly assisted reproductive techniques like IVF or ICSI .

To answer to your 2 questions -
1. No, I am not able to retract the skin. The skin remains at the tip of the organ.
2. The penetration is not difficult, though I need assistance from my partner since her vaginal cavity is a smaller. As a matter of fact, my wife got pregnant twice in recent years but they were miscarriages.
Ocassionally, the tip of my organ does swell, esp when I do the intercourse for longer period. At those times, I guess my wife also feels some strain (irritation while passing urine after intercourse). But in general I would say we do enjoy the union. I read that circumcision reduces sensitivity significantly ? Is it true? I am able sufficiently prolong myself at bed. Excluding foreplay, it is say 10min.
Secondly, thanks for your suggestion for IVF, ICSI. I wonder if IUI can be explored too?

There is a misconception in the mind of common man that Circumcision reduces the sensitivity ,
It does reduce the sensitivity for few days but it gets restored to normal gradually .On the contrary it delays and prolongs the ejaculation and thus helps to have a sex for longer duration making your partner happy and helps her to achieve orgasm.
As regards the problem you are facing due to Phimosis I would suggest you to get circumcised ,
In view of low morphology count IUI will not be of much helpful and successful .
In IVF and ICSI we concentrate the semen and select mature and normal sperms for fertilization .

Thanks for the inputs. I am more inclined to get circumcision done.
I recently got an ultrasound done for my scrotum, my urologist suggested that I do it since I was feeling some discomfort at times while sitting in office.
Thankfully, the ultrasound of scrotum and abdomen turned out to be normal. No varicocele or other such things. Testis show normal vascularity, texture is homogenous, no collections etc.
But the testis are as follows
Right testis in cm
3.9*1.9*1.6, vol is 6.7cc
Left testis in cm
3.3*1.8*1.9, vol is 6cc
I had once done a USG in 2011 for a mild hydrocele (cured by medicines taken for a week). The size is testes were
2.3*2.4*3.9, vol is 12cc
2.3*2.9*4.0, vol is 14cc
I am perplexed at the reduction in size. My doctor’s first impression was there might be an error. He physically examined and then told the testes are a bit smaller than normal.
But a 50% reduction has left me worried. Doctor said the sperm count (109mn per ml) suggests that it is not worrying. Need your opinion pls. Also is such reduction in size possible? Thanks in advance for your reply.

Thanks for uploading your scrotal Scan report which reveals the size of the both the testicles to be smaller than what was mentioned in scrotal scan done in 2011.
In previous scan there was a mild hydrocele which suggests it to be secondary to infection of both the testicles (Orchitis) which were inflamed and increased in size due to infection .
The size of the testicles is irrelevant as far as the function of the testicles that is secretion of Testosterone and sperm count is normal
There is nothing to be worried about .
