Hello Doctor! I Am Writing Because Of My 5 Month

Question: Hello doctor! I am writing because of my 5 month babt girl. From 2 days she has begin snoozing and had runny nose and cough. I gave her paracetamol and today I visited her to the pediatrain and she told me that the baby have bronchitis but for 6 days told me to do aerosol with clenil and physiologic water. after 5 days if she is not getting better I should start antibiotics. She has had fever today, XXXXXXX 37.8 and I gave twice paracetamol. It seems like a virus infection since I have been sick also. The problem is that is too litlle and maybe I should start antibiotics immediately. I am exclusively breastfeeding her. Please give me an opinion. Thank you

I have gone carefully through the details and understand your concern. Most often such episodes are due to some viral infections and usually they have an allergic component associated. Bronchiolitis is infection or inflammation of the bronchioles and is common in children of this age. Antibiotics are ineffective in viral infections like this. Irrespective of treatment it usually spontaneously resolves in 5-7 days. Treatment is supportive and usually comprises of antiallergic medication like cetirizine drops, and saline nasal drops for blocked nose. Steam inhalation or nebulization can help.

You need to give steam inhalation to your child. It helps in reducing the congestion and thereby the cough. Just sit with the child on your lap and place the source of steam before you. The water vapor will be in the air and automatically the child will breathe it in. Water vapor will reach deep inside and would help in diluting the cough and thus would provide relief.

If your child is otherwise active and playful, I would not be concerned. If not, it would be a good idea to visit a doctor and to get the child evaluated. If the respiratory rate is greater than or equal to 50 breaths/min you would need to take her to a doctor for the possibility of underlying pneumonia. If the rate is lesser, you need not worry.

If your child has fever with temperature more than 100°F, you need to give paracetamol to it bring down. Also use tepid sponging. If paracetamol is not effective in bringing down the temperature, you need to use Meftal-P (mefenamic acid) which is more effective. The minimum gap between two doses of paracetamol is 4-6 hours.

I hope that I could clear your doubts. Usually the symptoms resolve in a week or so.

Hello doctor! Today she seems to have less cough and the discharge from the nose is yellow but she has fever. She had fever 38.2 centigrade at 03 :30 Am after that at 4:00Pm and at 9: 30 Pm at 38 centigrade. I gave her paracetamol and I will see the doctor tomorrow to tell me if I should start antibiotics. With this fever do you think I should start giving antibiotic?

I understand your concerns but antibiotics may not help in a viral infection. Your doctor would be the best judge to check the blood counts. Antibiotics need to be administered only if the counts are raised.

Doctor hello again. Today till afternoon she was ok but now she is not feeling good. she had fever at 5 pm at about 37.3 and again at 8 pm. the distance between one episode and another is being close. i ask the pediatrain and she has dilemma to begin or not antibiotic. My baby is six days in this situation. Please give me an advice. thank you
