Question: Hello doctor. I have been undergoing prostatitis treatment for last 6 months. In April this year i started feeing better. My semen analysis showed 2-4 HPF however culture report showed pseudomonas aeruginosa. The doctor said since I am ok and there are no symptoms it is probably the sample contamination. He still gave me levoflox for 21 days and then asked to undertake one more semen culture and analysis.

I did the semen test again and the culture report was normal and the analysis now showed hpf as 8-10. I do not have those early symptoms anymore and there is very occasional pain in the scrotum which is of very less intensity. Do I need to be worried or you reckon that reports are normal and nothing to worry. Someone said that 8-10 hpf is not concerning and actually normal .

As per your description,it seems that you have been cured completely and you don't have to worry much.

Points in favour of this are...

1)You are free from symptoms now (Fever with chills and rigors,nausea,vomitings,urinary burning etc)

2)You have completed the full course of antibiotic treatment.(Quinolones like Levofloxacin are the drug of choice)

3)Your seminal culture is sterile (It is highly sensitive to detect a bacterial prostatitis).
This is an indicator that your infection has been treated adequately.

The reliability of semen as a diagnostic sample is very debatable.
Usually clinicians (and me) supplement it with urine test,Blood tests(counts /PSA) and sonogram to identify the disease.

I agree with your clinician on contamination theory because pseudomonas aeruginosa is the most common contaminent encountered in medical labs.

For good prostatic health,avoid activities that can irritate your prostate such as prolonged sitting,try to stand up as much as possible, be physically active, have a healthy diet, keep yourself well hydrated(that would keep the system flushed), practice safe sex, mantain good hygeine and avoid spicy foods.

My only question is about the pain in the scrotum. It is very short lived and does not affect me too much but is it normal when I touch or press the penis or wash it during shower.

Hi Sir.....

After the infection,inflammation of the prostate gland takes some time to resolve.
Many of patients complain of pain in the lower back,lower abdomen,pubic area, penis, scrotum and anus.

You can take NSAIDs like advil (Post meals) for pain and residual inflammation.

Many a times, muscle relaxants like Tizanidine and anti-anxiety medications are prescribed to the patients who do not respond to simple pain killers.

Prostatic massage (Massage therapy) can help drain the residual fluid that is causing the inflammation and hasten up the recovery.

Hot baths and heat therapies can also help to reduce the discomfort.

Make dietary changes,which I have advised(to avoid further prostatic irritation).
You symptoms should improve very soon.

Just one last question. For how much duration you recommend I should take that medicine.
