Question: Hello dr,earlier i asked you many question for my acid reflux.i think you have my full case history.i am on nexium 40 mg once in morning daily.i am raking it from 39th aug last month.i had acid eelfux very baldly and done with endoscopy and biopsy reveled mild reactive gastritis.i am feeling much better.burning sensation is still very less not continously.i want to ask u do i need to take domperidone for conplete relief.i am on diet change and lifestyle change completely.for how long i have to take medicine and my life style change.will i be completely cured.but i am feeling much better.what shiuld i do next to full recovery and these symptoms not occur in futrue also

Hello and Welcome to 'Ask A Doctor' service.
I have reviewed your query and here is my advice.

The only thing you need to do is to add a prokinetic drug like Levosulpiride or domperidone for 21 days. It would prevent reflux of acid formed in stomach back into the esophagus.

After 21 days we would taper you off the domperidone/levosulpiride and keep you on Nexium 40 mg twice daily (Esomeprazole has twice daily doses) while Rabeprazole 20 mg has once-daily doses. This should be taken for 3 months. After 3 months we would shift you on weak acid inhibitors like Ranitidine. We would taper off medications gradually after this or would try to keep you on minimum possible drug therapy.

Lifestyle modifications should be properly followed otherwise medications won't help.

In a nutshell,
First target is to control symptoms. Once symptoms are in total control we could proceed towards cure.
Nexium 40 mg once seems effective only for 12 hours.
Either make it Nexium 40 mg twice or Shift to Rabeprazole 20 mg once daily.

Pantocid it will also help me or not.i am also having throat irritation bcoz of cold so can i take cetrizine.will it cause acid reflux or not.after complete treatment and symptom free and complete cure can i come back to my normal life style

Yes you can take cetrizine.
Pantocid will do the same work take it twice daily in 40 mg doses.
Yes after recovery you could live a almost normal life.
