Hello, My Father In Law Recently Had Lab Work Done
Question: Hello, my father in law recently had lab work done which has revealed;
Kappa Lt chains 75.4, lambda 28.9, Kappa/Lambda ratio 2.61. BUN 49, Creatinine 3.12, iron 26, GFR 20.35, Ferritin 263.3, UA-mod blood in urine, trace protein, WBC 1-4, RBC 10.12, Bacteria 1+, urine culture showed no growth in 24hrs. serum protein electrophoresis shows alpha 2 1.1, M spike Gamma 14.1% g/dL 1.0, Ration A/G 1.0, biclonal pattern. Can you enlighten us to what we are looking at here? He has an appointment with nephrologist and primary next week, hematologist in February.

He is having elevated kappa chains, slightly elevated lambda chains, elevated k/l ratio, elevated BUN and creatinine levels, gfr is low. Also he is having elevated gamma globulins.
Most probably cause of this changes is any blood diseases most probably multiple myeloma or other types of blood cancers , Waldenstrom macroglobulinemia or other lymphomas .Also it is affecting the kidney function. I think that the next step can be a bone marrow biopsy.
