Question: Hello, my main signs/symptoms include frequent chest pain(tightness), upper back pain, shortness of while after a little exercise, nauseus which is common to me. Also, Fatigue and tiredness but those are common to me due to the nature of my job.

I would like to know more about your age and your past medical history.

Coming to this point, I would recommend performing some tests in order to investigate for the possible underlying causes:

-a resting ECG and a cardiac ultrasound in order to examine your heart function and structure
-a chest X ray study and pulmonary function tests in order to investigate for a possible lung disorder
- complete blood count for anemia
- thyroid hormone levels for thyroid gland dysfunction
- ESR and PCR for possible inflammation
- a cervico-thoracic spine X ray study for possible chronic backbone degenerations.

You should discuss with your doctor on the above tests.

In the meantime, I would recommend taking an anti-acid or PPI (omeprazole, pantoprazole, etc..) and see if your situation improves. In such case, a fibrogastroscopy may be needed in order to rule in/out possible gastro-esophageal reflux.

Another question, is the Anti- acid OTC or do I need a prescription or it is for specific OTC

Yes, the antiacid and PPI are OTC drugs. You can buy them without a medical prescription.

I would also encourage you to discuss with your doctor on the above tests.

Anyway, at your age, a cardiac issue would be really exceptional.
