I am writing to you because about four weeks ago I started to have sysmtoms of unexplained tiredness,
Changes in appetite, such as a loss of appetite or feeling full sooner.
nausea
Pressure in the pelvis and pain lower back.
A more frequent or urgent need to urinate and/or constipation.
So I went to my doctor on XXXXXXX 19th 2019 and they did a urine sample and he said I don't have a urinary infection but it did indicate blood in the urine. I explained my symptoms and he said that he would give me medication for nausea and a shot for my back pain. Said to come back if my symptoms got worse or did not get better. So I went to weeks later and my symptoms are still the same. I went back and he ordered a pelvic exam and x rays . I got my results and he said to come back in 8 to 10 weeks. as of today XXXXXXX 28th 2019 my symptoms are still same and nothing as change. But i made an appointment for July 1st. I also did have a menstrual period on XXXXXXX 14th and lasted three days. Today XXXXXXX 28th I had to go to the bathroom and as i wiped i noticed a bloody discharged. could it be possible that my period is starting again? should i get a second opinion. please help

I carefully read your query and understand your concern.

The bleeding is not menstruation in this case but it may be due to ovulation as you are 14 days after your menstrual period. It should not last more than a day in this case. If you bleed as you do with a period or it lasts more than a day, further evaluation is needed.

I would recommend you to have a TSH check if you have not had one in the last 3-6 months. Thyroid function may alter our energy level, may cause changes in appetite, bowel movement, changes in menstruation, etc.

You can also take movicol 2 sachets a day to help with constipation.
The ultrasound report attached shows a small cyst which normally is followed with a repeat ultrasound in 1-2 months to see if it regresses or grows, so I would advise you to follow the advice of the doctor to follow up on it.

As for the urinary symptoms, an abdominal ultrasound (the ultrasound you attached does not evaluate the kidneys) and an abdominal x-ray report are needed to make sure you are not having a urinary tract stone.

To conclude:
- repeat TSH blood test if you have not had it checked recently
- you can take voltaren 50 mg twice a day and buscopan 10 mg three times a day to help with the pain and add omeprazole 40 mg 30 minutes before breakfast to protect your stomach.
- make sure you eat frequent small amounts of food so your stomach is not empty and this will help you with the appetite as well.
- if no improvement with the above measures in a week or you notice any worsening you will need to see your doctor again.
- the mild bleeding 2 weeks after menstruation can be due to ovulation unless it is heavy bleeding or lasts from more than one day.
- kindly asked to upload the result of the x-ray and the abdominal ultrasound (that visualizes the kidneys) if already available
- you will need to repeat the pelvic ultrasound in 1-2 months to monitor the cyst noticed in the previous ultrasound

Should I be worry about the cyst? and whats a adnexal cystic lesion? ans what does it mean if blood flow is present ? is it just a simple cyst?

An adnexal cyst is a common incidental finding during a pelvic ultrasound and most of the time it is a simple cyst, especially if it is small in size as it is your case.

I would not worry about it as it very likely will regress in the next couple of months but you will need to follow it up to make sure it does not grow further but regresses.

If the cyst does not regress further tests will be needed to identify its nature.

The uploaded X-ray is one that visualizes the bones of the lower back and hips, it does not indicate anything about the kidneys. Sometimes kidney stones may cause blood with urine and urinary symptoms so I would recommend you to talk with your doctor if you need to have an abdominal ultrasound and an abdominal x-ray that will check for kidney stones.

As for "the blood flow is present", as I noticed it is said for the left and right ovaries and it is a normal finding and nothing to worry.
