I am Dr Sudhir Kumar, Neurologist, and would try my best to help you.
I have noted your clinical details. I agree that your symptoms are suggestive of benign fasciculation syndrome (BFS) and not ALS. However, severe generalized pain is not a feature of BFS. Mild pain may be present.
Secondary causes of pain should be looked at. Vitamin D is low, which needs to be corrected. In addition, other blood work such as CPK, calcium, magnesium, potassium, vitamin B12 level, thyroid profile, should be checked if not already done.
Cymbalta is good and the dose can be gradually increased to 60 mg per day. Other effective drugs for treating neuropathic pain are lyrica (pregabalin) and neurontin (gabapentin), which can be tried if cymbalta does not give adequate relief.
