Question: Hey Doc This has happened before but not this long. I have GERD if this has anything to do qoth this. I woke up at 6am because of this woth PAIN in upper stomach ( Solar Plexas area ) feels like I got punched there amd it hurts like hell. Any ideas??? please help. Thanks Aryeh

* As per my clinical experience, the severity of pain in upper stomach can be
associated with various possible conditions as acute gastritis, GERD flare
up, gall bladder inflammation or else.

However, I would like to know further details so that I can answer your query more precisely.

1. As you have mentioned, this has happened before, kindly give all details
of previous episodes.
2. Current pain is radiating to anywhere as back side or other area ?
3. Is there associated nausea, vomiting, altered bowel habits ?
4. Noticeable change in previous night dinner ?

Meanwhile, you can consume capsule omeprazole (20 mg ) or pantoprazole ( 40 mg ) with sips of water on empty stomach and tablet paracetamol ( 500 mg ) half an hour after that for best primary relief of symptoms.

Hi Doc Thanks for the fast response. The answers to your questions are below 1) It happened twice before in the past year and never before. The other times it was sharp as well but lasted for 10 minutes. This time for almost an hour and was relieved by a sudden belch ( burp ) 2) There was a little pain radiating to the back but MOST pain was the Solar Plexus area. 3) No nausea or vomiting The pain which I had was VERY painful and I actually am scared that it can come back opinion? Life style modifications
Diet maintenance
Exercises
Weight and blood cholesterol, sugar control
Supportive medications as proton pump inhibitor and bulk forming laxative
in case of constipation and anti spasmodic for pain scale of more than 3 of
10 score
Stress control
Quitting of alcohol and smoking if consuming

* Upon doing above with regular follow up on OPD basis, depending upon
the recovery I decide for need of an ultrasound of the abdomen & pelvis
if required.

Hey Doc I don't know if this has anything to do with my symptoms, however That night I didnt feel well and went to the ER. All blood tests were normal BUT I was VERY COLD and shaking. Today I woke up with 38 fever and now its at 38.4. Any ideas whats going on? Im not coughing, runny nose just hit and weakness. I took 4 asprins but still fever not going down. Opinion? Thanks

changes , which show up normal blood tests.

* Current weakness, low grade fever with runny nose indicate viral infection.
* As there are known problems with your stomach and intestine system, it is
inadvisable for you to consume 4 aspirins and mess up the system further.

* I would suggest to consume more liquids, steam inhalation and tablet
paracetamol ( 500 mg ) 3 times a day after meals preferably with antacid.

* The drenching in bed can be due to metabolic effects of over dose aspirin
or fever itself.

* Not eating for 2 days usually can not cause 3.5 kg weight loss in general.

* Obvious signs are of infection ( either bacterial or viral ) is of prime
concern as there is temperature and loss of appetite.

* A look through complete blood count is best advisable for this stage
evaluation.

Hey Doc First of all thanks for the processional advice. So update. I woke up this morning drenched in sweat meaning my bed was wet. Fever went down but varies from 36.8 to 37.5. Why is it going up and down ? is this normal? yesterday I took 6 Aspirin 325MG each . I also lost 3.5KG because of not eating much the past two days. Does this aound like a virus to u? Hi Dr XXXXXXX * The sweat I think is the fever as it went down but doesnt go to 38 anymore. * One day before the chills and fever I did a complete blood count with Liver and Kidneys, Magnasium and all was good. * Is there a reason why the fever goes up amd down? * Many times it happens with early phase of viral infection or else, in which
blood work up panel shows normal results as body immune system has
not launched that much detectable soldiers against the same.

* I personally rely more on clinical symptoms and the same are suggestive
of possible infection, you are requested to

- drink more liquids in sessions
- prefer easily digestible food stuffs with organic fresh fruits
- avoid spices, non veg, aerated drinks, dairy fat till possible
- avoid aspirin
- avoid stress, anxiety
- deep breathing exercises and YOGA

* Follow up after 24 hours about the progress or early as per the need.

Hi Dr XXXXXXX For my last question and thanks for your time. Would you suggest tp do another blood work? or is it enough? Thanks again for your advice.
