Hey Doctor, For The Last Year Now I’ve Had This
![default](/r/images/default.png)
For the last year now I’ve had this burning sensation and heaviness when I urinate. It’s not everytime I urinate but only sometimes but I never had this issue previously. I have had several urine studies done which have all come out negative for bacteria, infection or anything. I have also never had sexual relations at all so my doctor doesn’t suspect a infection I also visited a urologist who said the same thing. I originally thought it was the way I was masterbating in the prone position but I have since ceased doing that and switched to supine and it made no difference at all. One thing I do find with the burning pain if I masterbate that day I will get the burning pain for most of the day. Could this maybe mean there’s maybe a backflow of semen or it drying up causing the burning sensation is that a thing? The only other thing I can think of was last year I got a fleshlight and from memory around the same time as I started getting this burning pain. I no longer use it but could I have maybe not cleaned it properly and caught some low level some infection, bacteria or parasite I’m just not sure to think?

You may have chronic prostatitis
Detailed Answer:
Good day!
Thank you for being with 'Ask A Doctor' service...
I have read your problem and I'm sorry to hear that you have heaviness and pain on urination and that several visits to the doctor weren't able to help or detect your problem
I think you may have chronic prostatitis. This is usually caused by small amounts of bacteria that were able to go to the prostate and stay there.
Usually, for my patients, I suggest medicine as Tamsulosin (Flomax) help to relieve prostate inflammation. I would also get a semen culture or an EPS to determine if there are bacteria in the prostate.
In some patients, prostatic massage also help and could be done once a week for 2 months
I hope to have answered your questions. Please don't hesitate if you have any more follow up questions.
Regards

Thanks for the response I was also reading up about that being a possibility, with having that is that a bad thing that it’s gone on untreated for over a year now can this cause anything unreversable or cancer down the track? I’ve been to a specialist now and trying to actively fix the problem but have had no luck so far I might just go back and try to request those tests. Another thing is I was also on some heavy duty antibiotics for an ear infection a year back could that have maybe caused my natural Defense barriers to go down from that?

The infection won't cause cancer.
Detailed Answer:
Thank you for the follow-up question.
No being untreated for one year wouldn't have a much worse effect on you other than the chronic pain that you have been feeling. It won't become cancer or anything so don't worry.
Having the antibiotic for the ear infection wild definitely, kill the normal bacteria in the body but whether it would affect your prostate or not is hard to say and speculative at best.
If there are no bacteria then it is a possibility that antibodies may attack the Prostate causing infection. This is called abacterial chronic prostatitis.
Hope this answers your query.

Tamsulosin has few side effects. Plenty of fluids would go a long way.
Detailed Answer:
frequent ejaculation my help relieve prostate inflammation this is the same as doing a prostatic massage. what do you have to keep it not so frequent to give the prostate and and the ejaculatory ducts some rest. i would say ejaculation twice a week would be good.
tamsulosin has very few side effects and is not an antibiotic and it would really help you with the inflammation of the prostate.
lords of the timeline on how long it would take the treatment it would vary from patient to patient but the best thing is to get the semen culture to be sure if bacteria is living in your prostate from then if there is growth then it's just a matter of two weeks antibiotic eradicate the bacteria.
in the meantime i would suggest with drink around 2 to 3 liters of water a day avoid spicy foods and other spices that may aggravate their symptoms.
