Question: Hey doctor,

For the last year on and off I’ve been having this burning pain when I urinate. Pretty much every time I go it burns to some degree and I am unsure as to why that is. I’ve been to my gp multiple times and have done a bunch of urine and blood tests that show no signs of inflammation, infection etc and I haven’t been sexually active yet so it wouldn’t be an sti. I do however frequently masterbating once a day usually and I feel it’s worst after that which makes sense but I do also feel pain in the centre of my pelvic region where the prostate is when I pee. Is there anything else I can do or test for I have also seen a urologist who seemed to think it was nothing to worry about but it’s super annoying I also do have ibs but I unsure if that’s related to it?

Thanks for query .

Based on the facts that you have posted burning urination and pain in perineal region that you have since last one year is most likely to be due to infection of prostate (Chronic Non Bacterial Prostatitis).

This condition is characterized by all the typical symptoms of prostatitis like burning urination ,pain in penis and perineal region but there are no positive findings on all the relevant test carried out to establish diagnosis .
I would suggest you to consult your Urologist and request him to do following specific tests that will help to confirm the diagnosis
1) Semen Examination and Semen culture
2) Digital Rectal Examination .(DRE)
3) Microscopic examination and culture of urethral discharge obtained after doing DRE.

This is specific in the sense that you get secretions directly from the prostate and helps to rule out or confirm infection of the prostate .

In the mean time start following treatment
1) Tab Bactrim D S twice daily for 4 weeks
2) Prostate massage daily .This will squeeze out the prostate and will discharge prostatic secretions and will help to eliminate infection.(You will need cooperation of your close relative or friend to get him trained by health personnel to learn how to do it and do it at home everyday)
3) Reduce frequency of masturbation to thrice (weekly ).
4) Ensure to drink more water.

What actually causes prostatitis in the first place how exactly would I’ve have gotten this and how have I had it so long? Do you think this is even curable at this stage being that I’ve had it for so long now? I only just saw a urologist last week and he basically said not to worry about it then because there was no signs of infection through the urine tests. Is there any alternative treatment besides antibiotics as I have really bad gut issues and antibiotics make it exponitinally worse off would something like probitocs be able to treat it could it even be a yeast infection version of prostatitis?

Could the pain around my pelvic region also be because of my IBS? As I do feel the pain when I feel gassy or need to pass a stool.

The pain in pelvic region could be due to IBS You have to correlate the symptoms .

In Non Bacterial Prostatitis there is no positive evidence of infection on laboratory tests .It is based on clinical findings and symptom complex that patient have .

I have suggested the tests to be carried out and treatment in my earlier reply Please go through it and try to follow the same .

You will get relief from symptoms though not complete but to certain extent .

Hey doc one last thing is there much I can do to reduce the pain burning sensation? I’ve stopped masterbating which hasn’t really done anything it still burns especially when waking up in the morning.
