So I’ve been had digestive issues for the last 4 years but in the last 6 months my symptoms changed to mainly consisting of upper right sided pain plus epigastric pain, nausea and reflux where as before it was mainly just an upset stomach and reflux. But now the reflux tastes different and PPIs don’t work and from what I’ve read it sounds like bile reflux however I just had an abdominal ultrasound and I just had a HIDA scan today and the initial results seem to be normal but I have to see my specialist to get the final results. Can you still have gallbladder inflammation even if the HIDA and ultrasounds are normal I have a family history or gallstones but I don’t have any. I’ve also had gastroscopy/colonoscopy and blood work which were also all normal so what else could be cause the pain? I’m getting a distinctive upper right side pain that’s sharp and I can feel it when I breathe in too which is triggered after eating and lingers around. Heat packs and hot showers seem to ease the pain is there anything else I can try medications wise and what should I be eating and avoiding food wise?

If HIDA scan is normal along with a normal ultrasound I don't think the culprit is gall bladder.
Gall bladder opens up in duodenum while reflux of acid takes place from stomach which is above doudenum. Hence bile reflux is much more uncommon then GERD and with a normal HIDA and USG it's almost not possible to have it.
Two basic diagnosis of upper right quadrant pain is either gall bladder disease( ruled out in your case) or Acid peptic disorder.
Please try to understand that PPI is just a baseline drug for treating GERD rather I would say that just giving PPI is a incomplete treatment in itself.
Why?
GERD (Gastro esophageal reflux disease)-
Acid secretion is controlled by PPI's but what about the second component Reflux?
No drug has been given to control it.
Either levosulpiride or Domperidome should be added to PPI's.
Secondly which PPI were you on and what was the dose?
For example doses of Esomeprazole ranges from 20 mg twice to 80 mg twice daily.
So unless and until patient stops responding to maximum doses one can't say that medicine is not effective.
Thirdly Magaldrate syrup to your regimen. A very good antacid.
Now let's talk about diet.
In control of acid peptic disease the biggest game changer is life style modification. Harsh but truth is that no amount of medication would help you if you won't have a diet which suits you.
For e.g you have got a lacerated wound on your hand...No doubt you apply wound healer daily but you scratch the wound twice a day..Would it heal?
Answer is no.
The same is true with acid peptic disorder.
On one hand you take medicine to suppress acid formation while on other hand you are taking food which is junk and needs lots of acid and other enzymes to digest it and even sticks to intestinal wall and acts as a breeding ground for yeast and bacteria. In such case medication won't help.
Solution-
Eat clean light food 70% of times
Rest 30% of times you could have cheat meals ( please don't binge).
Initially make it 80:20 then slowly and slowly try to bring it to 50:50 ratio(If you really can't control your junk requirements).
Secondly right now you need to detoxify your body.
Eat 50% food as raw( fruits nuts salads) and rest 50% as cooked.
Food should be very light low in fat and should be home cooked.
Take Kawa roots extracts for anxiety.
You can eat all vegetables.
If possible no flesh for few weeks.
Drink a cup of chilled milk(low fat) in morning.
Take some saffron and leave it overnight in water. Next morning drink that water. Saffron has excellent anti inflammatory property.
In case you want to know about herbal medicines ( effective ) please revert back. They are usually helpful in detoxification with a proper diet.

Along with it take isabghul husk 10 gram three times a day with chilled milk.
Milk is alkaline in nature while stomach is acidic. So it would neutralise it.
Take a probiotic daily.
Leave non veg or any heavy food for 3-4 weeks.
Take saffron with boiled milk.
Drink plenty of water but not forcefully( Drink only when your body feels thirsty).
Don't over think.
Take Medhavati(Herbal available online two tablets a day to calm your nerves).
No alcohol,tea or coffee.
Eat lighty cooked vegetables.
Avoid too much dairy.
Yoghurt may be taken.

Flare up of symptoms are due to change in food.Even food served by airlines are spicy and oily I hope there is no need to comment about restraunts.
Your symptoms are typical of acid peptic disorder (chronic in nature) you need a well balanced home cooked light food for few months with proper medications.
Already too much of testing has been done dear.
Follow my suggestion you would start feeling better soon.

It's not H.Pylori( though you can go for tests).
