Question: Hey doctor,

So for about 6 months now I’ve been having this burning sensation when I pee. I’ve been to my doctor and I’ve been through numerous blood tests, urine tests, ureoplasma test and even a swab of my penis with everything coming out normal. Now what I’ve also noticed when I do get this burning sensation is white looking sediment coming out. I thought nothing of it at first but when I masterbated the other day I noticed the exact same sediment when I went to pee. So it’s got me thinking is my semen some how leaking out into the penis even when I’m not ejaculating forcefully causing this burning pain when I pee? I find if I drink a lot of water the sediment eventually clears out and doesn’t burn but as soon as I get an erection or masterbate it is back. Could this be a possibility about what I have disucssued or could it be my prostate at my age? iIt’s nof a huge issue but it does bother me that’s for sure. Is there any treatment for this or other tests I should do?

Well, I think you meant to say prostatic enlargement which is seen among the elderly group of people, however, infection and inflammation of the prostate can happen at any age...

Yes, the white sediment could be semen only, leakage can happen due to straining or long term irritation and infection...

Since the blood and urine tests were normal, at this outset, there could be some other possible causes, starting from a prostatic infection/inflammation, a resistant urinary tract infection, ureteric stone, if you're sexually active then some specific group of STDs (Chlamydia and Gonorrhea) should also be investigated for...

To specify the diagnosis, a thorough re-evaluation by your Physician/Urologist along with a few relevant investigations are necessary (namely Prostatic secretion microanalysis, and culture sensitivity, serum PSA levels, repeat urine routine microscopy for chlamydia and gonorrhoea, culture sensitivity assay, blood test for chlamydia and gonorrhea, Ultrasound KUB and prostate) to look into it further...management can be done based on the findings...

Please discuss this with your doctor and proceed further...

Meanwhile, please drink plenty of water (10-12 cups per day at least) and cranberry juice to help to relieve the symptoms...Avoid irritant foods like too much coffee, tea and carbonated drinks, some acidic fruits i.e. oranges, grapefruits, limes, spicy foods for the time being.

Hey doctor thank you for the reply I’m not sexually active and that’s already been ruled out with tests just go check anyways. With the prostate issues what can usually cause that and is it bad that it’s been going on for so long now what sort of damage could that have done? I’m going to go back and enquire about it with my doctor but it’s quite annoying that they missed it and I only just recently re did the urine tests last week so I guess the prostate specific tests are the only thing I haven’t done I had also completed a urinary tract ultrasound a month ago when I was still having symptoms so I assume it’s still relevant. If it were prostate issues what treatment would usually be issued and would it be worth going straight to an urologist or sticking with my gp for those tests would they be able to do them?

Well, it's good that all other tests have been performed already...Yes, the recently done ultrasound should be relevant in this context (if it was also focussed on the kidney, ureter, bladder and most importantly prostate)...

Yes, the prostate-specific tests (Prostatic secretion microanalysis, and culture sensitivity, serum PSA levels) should've been done already since your complaints are still persisting...the extent of damage can only be ascertained after confirmation of the exact diagnosis...

Treatment depends on the findings mainly, generally, it involves broad-spectrum antimicrobial therapy (antibiotics/antifungal/antiprotozoal based on the findings) along with urinary desensitizer, antispasmodics...

It seems like an ongoing issue for quite a while now, so I'd suggest going to the Urologist directly in this scenario...However, if it's taking time, you can directly approach to your GP for a quick referral to the Urologist...
